Caught in, under or between a moving and a stationary object

Employee was performing maintenance on a v-belt and sheave assembly when EE's right thumb became entangled between the belt and sheave resulting in the right thumb being severed.

Struck by falling object

Employee wrongfully entered a permit required confined space by self to clean material that had accumulated inside the chute. A large chunk of fines broke free and struck the employee in the lower left leg causing fractures to both lower bones in the left leg.

Over-exertion in pulling or pushing objects

P5 best stopped loaded with rock causing the drive belts to burn off. Employee was shoveling/raking rock out from underneath the belt and felt a sharp pain in EE's left shoulder.

Struck by... (Not Elsewhere Classified)

Employee was asked to move water pumps. EE drove EE's service truck to the first pump. As EE reached down to pull the ring on the cam lock, the ring pulled opened and sliced the employees left middle finger. Requiring stitches to close the laceration.

Struck by flying object

While performing maintenance on the primary crusher with the hood open the employee received an FBE in left eye. The employee did not report the incident until 9/4/2019 resulting in the FBE being embedded in the eye. The employee was wearing safety glasses and a face shield while grinding.

Fall onto or against objects

Employee was working in feeder area. Employee lost footing, fell forward, and struck forearm, causing a fracture.
