Fall from machine

Installing engine on Bulldozer. Climbed up on push arm and rt Ft slipped and fell to the ground catching himself with his right hand and arm and jamming his rt shoulder. Saw Dr., taking MRI and PT.

Struck by... (Not Elsewhere Classified)

The employee was working on the feed plate on the gravel plant screen. He was using a drive ratchet to tighten the bolts on the plate. The bolt slipped causing the ratchet to strike the employee under the chin. The strike resulted in a laceration approximately one inch long and 1/4 inch deep.

Fall onto or against objects

Employee was dragging a piece of 8"" PVC tubing attempting to attach it to the water pump on a barge and stepped backwards into a gap between the barge and catwalk. He fell down and struck the backside of his left upper arm on the corner of the handrail which caused a laceration that required five stitches.
