FALL DIMENSIONS-20' L X 20' W X 4' HIGH. OCCURRED IN INTERSECTION OO2 SECTION 1 XC INBY #1533 ROOF SUPPORT 4' FULLY GROUTED RESIN BOLTS
A roof fall was discovered in the last open crosscut of the #4 entry 1 block inby survey spad no. 1454 in the mains 001-0 MMU section. The fall measured 65' long, 20' wide and 10' - 12' in thickness.
Highwall fell onto the #2 entry canopy, damaging the canopy and pulling out 4 roof bolts in the entry.
The employee was changing scoop batteries & walked into a low hanging roof bolt, causing him to strain his neck.
While walking to get a knife, employee back hit the head of a cable bolt installed in the roof.
The injured employee was operating a mobile bridge unit at the time the incident occurred. The canopy of the bridge unit caught a roof bolt and stopped suddenly, jolting the employee. (Employee notified foreman on 7/21/2005 of injury.)
EE WAS ATTEMPTING TO LIFT MOTOR FROM MANTRIP TO PARTS TRAILER AND TWISTED HIS BACK.
SCOOP OPERATOR WAS CLEANING #6 ENTRY & LOST BRAKES ON SCOOP. RAN INTO COAL FACE & TWISTED BACK. BRAKES WERE WORKING PRIOR TO ACCIDENT.
The injured was roof bolting with a twin head Fletcher, operating left side and was in the process of swinging the head of the roof bolter toward the left rib to install his first bolt in that row, when a piece of head coal approx. 4"" thick x 30"" wide x 24"" long fell from between the bolted area hitting him on his left shoulder.
This individual was operating a 488 S&S scoop. He had pulled the rubber-tired supply car into a crosscut. The chain was hooked to the bucket of the scoop and frame of the car. He raised up the bucket to get slack in the chain to unhook it. This caused the car to drop and smash his right thumb between supply car and bucket.
The individual was operating a twin head roof bolter on the 002-0 section, 2rt crosscut. he had finished installing the 2nd EE inside bolt and was dropping the safety canopy down when a piece of rock approx. 12"" long x 6"" wide x 0-1"" thick bell off of the canopy striking his rt index finger which was resting on the drill head.
EE WAS PUTTING BOLT IN HEAD OF BLOTER WHEN A PIECE OF ROCK 2 1/2"" X 24"" X 24"" FELL ON BOLTER SMASHING MIDDLE FINGER
The injured was watching a mechanic pry a bearing off of an axle of 3 wheeler battery mantrip, when a piece of the bearing housing broke off striking his rt. hand
Back sprained while duck-walking carrying 40# of required equipment thru all entries of underground mine.
EMPLOYEE SUSTAINED AN ALLEGED EYE INJURY. TRIED REACHING EE ON SEVERAL OCCASIONS STARTING ON 9/26/05, WHEN STATE MINE INSPECTOR INFORMED 1A MINE (AFTER EE CALLED HIM ON 9/25/05). SPOKE TO HIS WIFE ON TWO OCCASIONS BUT INJURED EE HAS NOT RETURNED ANY OF THE CALLS.
Subject was attempting to back scoop around right corner and had his fingers in pinch point area and as a result his fingers were smashed but not broken.
Backing up, bridge operator left carrier to move miner cable. Light cord caught on left tram lever causing machine to swing sideways. Deck went on top of right foot.
