At approx. 1:45PM the hoist became inoperative the section was notified and production was stopped. After investigating it was determined that there was an electrical problem. The elevator was down approx. 2 and half hours. No persons were trapped or endangered during this stoppage.

While checking a pump in the return at approx. 9:25AM a certified examiner identified a roof fall. The fall was located in the 2 section return #1 entry about 80 blocks outby the section near spad#3433. The fall measured 17'W X 16'L X 7'H and was supported with 5' fully grouted bolts. The area has been dangered off and timbers set. The area will be cleaned up.
16 block on #6 belt had a house back fell out exposed bolts
The computer system needed rebooted to allow the emergency hoist to operate.
The hoist rope broke on the emergency escape hoist during weekly inspections
There was a low voltage drop on the mon power feed line that caused the elevator to need to be reset.
While drilling shot holes in the mine floor in the #4 entry on the #1 Section with the RRII Fletcher roof bolter a pop occurred in the mine floor. The injured employee was struck with rock that had been blown out of the drill hole.
While drilling shot holes in the mine floor from the right side operator station in the #4 entry on the #1 Section with the RRII Fletcher roof bolter a pop occurred on the left side in mine floor. The percussion from the pop knock the employee down.
At the end of his shift the employee was crossing over the bridge and lost his footing and fell. This caused his leg to be pinched between the bridge and Lo-Lo causing a contusion. The employee was treated and released and remains off at this time.
The employee was cleaning a rock box at 2A head drive when he fell onto the # 11 belt after it inadvertently started due to not being blocked from motion. The employee was diagnosed the hairline fracture to 3 vertebrae's in his neck.
The employee was riding on a mantrip when the mantrip hit a supply trailer parked in the crosscut. The employee suffered a contusion to his right leg. The employee followed up with a physician on 2/10/2015 and was put off work.
Employee was changing a belt stand out on #2 belt at 19 block when a piece of draw rock fell. The piece of rock struck the right side of his hard hat and right cheek this caused a laceration that required sutures. The employee was treated and released to full duty NO days were missed.
Employee was roof bolting in the #2 to #3 crosscut on the #2 Section when a piece of roof rock fell striking him in the back of the head creating a laceration that took 2 sutures to close.
