Accident type, without injuries

A roof fall occurred outby in the west mains #1 along the #9 belt in the #5 entry return air course located at spad # 0593. Fall measured 25W 15H 30L.

At approx. 10:45pm a roof fall occurred on the No.2 section in the No.1 face measuring 17'W 25'-30'L-5'H. This area had already been cribbed and dangered off due to the current roof conditions.

At approx.9:40A.M. on the #2 section a roof fall occurred in the #5 entry. The fall was approx. 20'W X 60'L X 5'-6'H the area was supported with 5' fully grouted resin bolt and supplemental 10' cable bolts. The area will not be cleaned up the area has been dangered off and cribs have been set.

Approx. 10:30A.M. on the #2 section in the intersection of #2 entry a roof fall occurred. The fall was approx. 20'W X 20'L X 5'-6'H the area was supported with 5' fully grouted resin bolts and supplemental 10' cables bolts. The area will be cleaned up to recover a Fletcher bolter that was covered up in the fall. The area has been dangered off and cribs set at all approaches.

A roof fall was reported in the old 3rd left B-3 panel, one break inby spad 2047.

Caught in, under or between a moving and a stationary object

The section crew was adding a 3rd bridge when the power cable drug under the 3rd bridge. The employee signaled the 3rd bridge operator to stop and when he did, the employee reached under the bridge and grabbed the cable. The 3rd bridge tilted and pinched the employees hand under the bridge.

Struck by falling object

Employee was riding on a 10-man rubber tire mantrip when the mantrip lost its brakes and drove through a set of equipment doors. The employee raised his feet to keep the door from falling on him and his right foot was struck by the top of the door, breaking bones in his foot.

Fall to the walkway or working surface

Employee was duck walking in the #3 entry when he tripped on a rock, which has been scaled down from the top, and fell to the ground breaking his leg.

He was assisting one of Carter Roag's employees tighten a chain on a conveyor when the grease hose blew off putting grease in his face. He asked the employee of Carter Roag to throw him a rag as he could not see, he removed his safety glasses to use the rag and the rag that was gave to him was dirty and when he wiped his face and eyes it cut his eyeball.

Struck by falling object

Employee was scaling the top when a rock fell and hit him in the left leg. X-rays taken at St. Joseph Hospital were negative and the employee returned to work on his next scheduled shift. The employee continued having pain and was seen by an orthopedist, which recommended surgery on September 25, 2014. *The Employee was operated on September 25, 2014 and will be off work.

Employee was helping carry Jack leg drill. His left leg gave out. Pre-existing condition pins popped in leg.

Struck by falling object

Employee was bolting a brow in the #7 entry. The starter steel was in the brow about 2"" when the brow fell out and caught the employees left hand between it and the drill head, lacerating his thumb.

Over-exertion in lifting objects

Employee was picking up a 15HP pump when he felt a muscle pull in his stomach. The employee saw his family doctor the following day and was diagnosed with a hernia.

The employee was walking to the stacker belt when he stepped in a hole and hurt his left knee. X-rays taken at Davis Memorial Hospital were negative and the employee returned to work on his next scheduled shift. The employee continued having pain and was seen by an orthopedist, which recommended surgery on Monday July 7, 2014. *EE was operated on 7/7/14 and will be off work.

He was working on a carrier haulage system when he bent over and his back went out.

Struck by falling object

Employee was taking drill steel out of the top. he dropped one of the drill steels & when he went to pick it up another steel fell out of the top & hit the employee on back of the head.

Employee was cutting belt with a utility knife when the knife slipped and cut his left palm.

Employee was installing springboard on the miner with a slate bar. The slate bar slipped and caught his right index finger between the slate bar and frame of the miner.

Employee was walking through a check curtain and the wind blew the curtain into his face, causing debris to blow into his eye.

Struck against stationary object

Employee was bent over walking through a curtain. He stood up and hit his head on a belt hanger, causing a laceration.
