At 2:25 AM it was reported that our elevator was down. No one was trapped or injured. The problem was that a resistor for the drum brake had blown. The resistor was replaced and this corrected the problem.

Brakes wouldn't release, so attempted to adjust the brakes and that did not correct the issue, so they de-rated the weight capacity to correct the issue.
The elevator went down at 11:10 PM. Changed the cab door switch to correct issue.
Cage safety dog switch wire broke in trailing cable, plugged into spare in trailing cable to correct issue.
The network switch was bad. Corrected the issue by replacing switch. There were no injuries that occurred, and the lifeline at the bottom was switched around to exit mines on P-Hill side. The lifeline was switched back to exit at elevator once switch was replaced.
Elevator went down at 5:00PM. Wire broke in control cable going from PLC to the cage, for cage e-stop. The mechanical brakes were glazed. FK Lakeshore cleaned brake drum and tensioned the mechanical brake. Also changed parameter settings. This corrected the problem.
The elevator would not drop automatically. The elevator was fixed by replacing a contactor.
At 12:50 AM it was called out to dispatcher that our highwall at Pleasant Hill #5 entry had a rock slide down on top of the canopy. It was not impeding the air and was sitting on the cribs. It was found by an employee and an MSHA Inspector. The area was dangered off to prevent anyone from entering area.
It was reported that the elevator was stuck on bottom with the doors open. Nobody was trapped or injured. The cable holding the counterweight for the outside door had broken. The cable was replaced and the counterweight was re-attached so doors would operate properly.
At approx. 9:10 on Apr. 15 the cab switch would not allow the door to open. During repair efforts it was determined that a build up of grease and dirt kept the latch from reaching its release point. The hoist was down approx. 80 minutes. No persons were trapped or in danger during this time.
Employee had set the CM up in #6 entry. EE was turning EE's body around to get better positioning, as EE turned EE tripped over a lump of coal and fell to the ground. this resulted in a fracture to EE's right elbow. EE has since had surgery to correct the fracture in EE's elbow.
Changing jack on left side bolter, the long arm on the bolter broke free and hit left arm. This resulted in a broken bone 4"" back from the thumb.
Employee was installing first row of 5 foot roof bolts in the #8 face / heading. Employees reached into the air to tart steel and a piece of draw rock fell and smashed EE's hand on roof bolter. Resulted in a broken left pinky finger. Employee was wearing the required PPE.
On June 29, 2016 at approximately 10:10 PM a foreman reported that they saw an ignition in the crusher area of the Cogar Feeder/Crusher Serial #10-035 located on 1 Section at the tailpiece of #16 belt. The material in the crusher at the time of the alleged ignition was sandstone.
Employee was hanging Shuttle Car cable to a cable hanger suspended to the roof and felt a pain in groin/back. Employee was transported to hospital. X-rays were negative but was placed off work until 5-23-2016.
EE was operating a Fairchild Scoop. While entering the #7 entry LOC on 1 Section the EE used left hand to move the line curtain away from the scoop deck. EE's foot then slipped off of the brake pedal and the scoop drifted forward toward the face catching EE's left hand between the scoop deck and coal rib.
At approx.10:00PM on Section 1 in the #5 entry a section foreman knelt down to nail a back check down. While knelt down EE notice a piece of equipment coming through the back check. The foreman yelled and lunged to get out of the way and fell down. The foreman complained of pain in lower left leg. EE was transported to St. Joseph where an x-ray showed a fractured Tibia.
