A roof fall was discovered by an examiner in a worked out area of the mine on the 4G Panel, in the old belt entry, one block inby spad 9561. There was no interruption in the ventilation and no equipment involved.

Accident occurred between 6/29/12 at 7:45p.m. and 7/2/12 at 1:00a.m. when carbon monoxide was detected and damage to the #39 seal water traps was observed by the mine examiners.

Fall from machine

Pulled truck off scales went back to scan card. Right foot slipped on step while holding handrail. Fell and twisted left knee and pulled right shoulder.

Employee was operating a roof bolt machine. After he drilled 3' of the holes, he dropped the pod to add a piece of steel when the head dropped it came in contact with the drill steel in his right hand, which was resting on his thigh causing the drill steel to penetrate his thigh causing a laceration.

Employee was cutting the bottom of a Mega Dust Bag with a knife, when the knife slipped and he cut his forearm.

While removing section of heavy media pipe (approx. 10"" dia. X 24"" length) EE went to push on side pipe as it was being removed it came out swung around striking him on the right wrist/forearm, breaking wrist. (closed fracture)

Fall onto or against objects

Was sitting on picnic table. When he got up he lost his balance and grabbed wall to keep from falling and hand landed on a screw in wall and cut his hand.
