Accident type, without injuries

A roof fall was discovered by airway examiner on C1 panel in old worked out area. The fall was at #3 left entry spad # 2227 in 4 way intersection. Area has been timbered off. The fall was 25' wide X 40' long X 6' high. It was found at 11:15 pm on 12/18/08.

A roof fall at 2nd left mains panel I at intersection of spad # MP 12 of old works. No ventilation problems. Was found on weekly airway travel. Area has been timbered off.

A fall occured in belt entry without any warning. Approx 60' long and 12' high. Inby Spad 4904.

Fall to the walkway or working surface

Lost footing in mud and fell. One foot was too deep and didn't give when fell.

Struck by falling object

Was bolting in #3 entry and while drilling, 8"" rock blew out and struck his knee

Over-exertion (Not Elsewhere Classified)

The injured was walking around the bucket of the parked endloader in order to get in the machine when his right foot slipped on a patch of ice in the roadway and his left foot remained stationary. This caused his left foot/leg to twist and resulted in his full body weight to shift to his left foot/leg causing a fracture just above the ankle.

Fall onto or against objects

EE was pulling belt with channel locks and changed locks, slipped and EE fell into rib and hurt right shoulder. EE had to have surgery on 12/8/08. Returned to work on light duty 12/9/08.

Struck against stationary object

EE was putting first aid box in scoop on power move and caught rough edge with right thumb.
