Accident type, without injuries

Area worked for awhile and then fell, approx 12' high and 100' long in #3 belt entry at spad #6840. No equipment was involved and no interruption in ventilation.

Fall to the walkway or working surface

EE was walking to mantrip on down hill grade and slipped on pavement and fell on his rear-end.

Flash burns (welding)

Injured was lying under bridge and was trying to get the welder to arc and didn't have shield in place and got flash burn from welder.

Fall from machine

Was crossing bridges and fell striking knee on pavement. Injured on 6-7-10 and returned to work 6-8-10 and worked until 7-28-10. Now he will be off until after surgery.

Caught in, under or between a moving and a stationary object

Was pulling a rock off side of miner and rock smashed tip of finger between rock and frame of miner.

Struck by falling object

EE was standing beside belt and a piece of top structure came loose from belt and struck him on the left hand.

Fall onto or against objects

EE was shoveling gob out from behind ram of the scoop. EE slipped hitting his ribs on the ram of the scoop.

Over-exertion (Not Elsewhere Classified)

Was lifting up and pushing fork on fork lift and felt pain in left arm. Needed surgery to repair damaged ligaments, received 3 stitches.
