Accident type, without injuries

Notified on Wed. 4-6-05 @ 3:45 am that 1450 BE Dragline located @ Jeddo Basin was on fire. Dragline was idle @ time of fire & no EEs were working at Jeddo Basin @ time of fire. Fire was extinguished by Hazle Township & Freeland Fire Companies @ app. 4:10 am, having rcvd fire call at app. 3:24 am. There was property damage to cab area & direct current control room.

Fall to the walkway or working surface

WALKING IN OUTSIDE AREA OF WELDING SHOP, AFTER SNOWFALL - SLIPPED - TWISTED RIGHT LEG - FELL.

Employee was leaning on pickup truck front fender. Other employees were utilizing pickup truck for pump repairs, caused pickup to rock. Employee claimed rocking motion of pickup cuased him to bump rib area on fender. Pickup was not in operation at time of incident. Did not seek treatment until 11-6-05. Reported to company on 11/7/05.

Over-exertion (Not Elsewhere Classified)

Dismounting crane, stepped into ground depression, twisted ankle. Did not seek medical attention until 10/31/05. Returned to work, no lost time & no restrictions. Re-check on 11/2/05 for continued pain. Time lost starting 11/4/05.

Over-exertion in lifting objects

Lifting wire cable to cut trip ropes for draglines-musce strain hower back, stomach, groin and r side.

Lifting pump casing-casing struck right knee-had pain and swelling no time lost and no restriction-rechecked because renewed pain-mri showed a tear in muscle went off work on 9-22-05 to have tear repaired.

OPERATING ROLLER, EMPLOYEE TWISTED HIS BACK. INJURY OCCURED ON 5/5/05 & WAS NOT REPORTED TO THE COMPANY UNTIL 5/12/05.

SHOVELING EXCESS MATERIAL AT RAIL LOADING SITE. DID NOT SEEK MEDICAL ATTENTION UNTIL 03/03/2005.

Struck against stationary object

Reaching to open valve - cut top of hand on clamp. Sutures required.

Struck by flying object

Grinding on metal with 4"" angle grinder - foreign material entered left eye. Accident occurred on 9/2/05 - did not seek medical attention until 9-6-05.

Welding on dragline bucket, foreign object in left eye.

Inhalation of radiations, caustics, toxic and noxious substances

DIAGNOSIS OF COAL WORKERS PNEUMOCONIOSIS ON 9/26/05. LAST DAY WORK 9/30/05. HAS NOT AS YET BEEN AWARDED BENEFITS.
