Over-exertion in lifting objects

Moving 55 gal barrel. Injury occurred 12/21/10. Rechecked on 3/16/11 - off work since 3/16/11.

Fall from ladders

Standing on ladder, EE slipped and fell to the floor. Landed on buttock. Acc. occured on 10/21/10, did not seek medical attention until 11/12/10. EE has been off work since 11/12/10.

Over-exertion in lifting objects

Moving hoist chain & felt pain in right groin & side.

Climbing ladder onto haul truck - felt pain in neck. Accident occurred on 6-15-10 - did not seek medical attention till 7-16-10. Off work starting 7-19-10.

Struck by flying object

Taking bobcat out of garage - wind blew foreign material into eye.

Contact with hot objects or substances

Cutting pipe w/torch overhead, dropped torch and burning tip entered glove, causing burn.

Over-exertion in lifting objects

EE experienced chest pain while shoveling material. Pain seems to be heart related as no injuries were reported. Further testing as outpatient is scheduled for heart condition. Tests showed no blockage but irregularities to the vessels were noted. Also drinking power drinks may have brought on symptoms.

Fall from machine

Replacing screen on sieve. Weld broke on rail, fell to floor, injuring back.

Non Chargable. EE was found slumped over inside the water truck which he was driving. According to reports, EE suffered a heart attack brought on by several blockages, including one blockage that was at or close to 100%. EE died on 9/11/10 after life support was disconnected.
