A fire occurred on the front end loader. The operator (victim) was dumping a bucket of refuse rock on a stockpile. A nearby truck driver noticed an oil leak on the loader and radioed the victim to shut down. The victim dumped the loaded bucket, backed up and drove the loader forward. The loader burst into flames. Fire was located between operator's cab and engine compartment.

Stepping onto 120% wabco haul truck deck-twisted left ankle.
EE WAS MARKING DRAG ROPES WITH SPRAY PAINT. HE STEPPED ON WALKWAY AND HIS RIGHT FOOT WAS NOT FULLY ON THE WALKWAY. THE BACK OF HIS FOOT FLEXED DOWNWARD, CAUSING A STRAIN TO HIS RIGHT ACHILLES TENDON.
Trouble shooting electrical problem, stepped into inspection opening.
CLEANING TRACKS ON DOZER, TRIED TO DISLODGE ROCK STUCK BETWEEN TRACK & FRAME. WHEN TRYING TO LIFT ROCK HE FELT PAIN IN HIS RIGHT LOWER BACK.
Moving electrical component on hand truck, weight shifted causing strain to right shoulder. Accident occurred on 4-28-04, did not seek medical attention until 5-10-04.
EE WAS LOADING STEEL GRADING AND PINCHED HIS RIGHT HAND BETWEEN PIECES. LACERATION TO HIS MIDDLE FINGER, RIGHT HAND - REQUIRED SUTURES.
