Injury occurred 3/15/16 - unable to work beginning 6-27-16

Over-exertion in lifting objects

Injury occurred on 3/4/16, did not go to the doctor until 3/10/2016. Off work since 3/11/2016. Removing covers from generator set, twisted left knee causing sprain of ligament of knee and aggravating osteoarthritis.

Caught in, under or between a moving and a stationary object

Changing bushing in blade tilt cylinder - cylinder shifted pinching finger between bushing and blade arm.

Struck against stationary object

Trying to move wheelbarrow, wheelbarrow was frozen to ground, employee forced wheelbarrow out of frozen material, when it broke free employee fell backward striking EE's lower back on a board.

Struck against a moving object

Injury occurred on 10/16/16 -did not receive MRI results until 11/11/16 - working but receiving PT. Driving pickup - vehicle veered off to side of road - struck rock with under carriage - bounced arm on center arm rest resulting in stress fracture.
