Walking to dragline slipped and fell

Replacing belt on vibrator - pinched hand between belt & pulley

Changing belts on crusher finger caught between belt and sheave. Avulsion requiring sutures.

Working on manlift to install new house crane in 8700 dragline. While setting limit switches for crane operation, check valve on manlift basket malfunctioned causing basket to tip. Wearing the proper PPE (harness) prevented him from falling, although the jarring action caused injury to left shoulder.

Twisted leg getting off dragline. Pulled arm to avoid fall. Accident occurred 7/31/12. Off work since 8/14/12.

Cutting tooth wedges on drag line bucket w/ torch hot slag fell onto shoe burning hole into shoe and burn to top of left foot. Accident occurred on 6/19/12 did not seek medical attention until 7/2/12 off work starting 7/3/12

Working on spray bar w/pipe wrench, wrench slipped pinching finger between wrench & metal plate.

Walking in dragline tripped/slipped on grading fell on right knee. Did not seek medical attention till 4/2/12 off since that date.

Climbing onto haul truck front end loader bumped truck causing movement of truck jarring EE. Treatment date 3/26/12 off since 3/27/12.

Walking toward vehicle slipped in mud - jammed right arm against vehicle - fell back and grabbed vehicle - pulled arm. Did not seek medical attention until 11/8/12 - off since 11/8/12.

Bending under mirror banged hat on mirror forehead hit bracket bolt causing laceration to forehead requiring sutures.

Diagnosis w/carpal tunnel both hands on 7-31-12 did not seek treatment until 3/4/13
