Doing regular job - experienced pain in right shoulder area. Partial tear of right bicep.

Moving grease barrell, felt pain in right abdominal area. Abdominal Incesional hernia. Did not seek medical attention til 10/24/08. Off work starting that date.

Fall from machine

Climbing front end loader ladder, he slipped and fell to the ground.

While repairing water pipe, moved to avoid water, twisted causing pain to lower back. Accident occurred 8/19/08. Did not seek medical attention until 8-25-08. Off starting 8/25/08.

Over-exertion in wielding or throwing objects

EE was swinging a sledge hammer, developed pain in neck area.

Repairing coal crusher and was removing his burning goggles, EE had a foreign body in his eye. Did not seek medical treatment attention until 6/6/08.

Removing I beam from dragline. Pinched little finger between I beam & dragline motor. Sutures required.

Swinging hammer, finger pinched between hammer handle and pin causing laceration to left index finger requiring sutures.
