Struck by falling object

LIFTING 2"" X 20""BOLT OUT OF DRAG BEARING CAP, USING HOUSE CRANE & SLING. BOLT BECAME JAMMED, EMPLOYEE GRABBED BOLT & THE SLING BROKE CAUSING THE BOLT TO FALL STRIKING RT. MIDDLE FINGER TIP. CAUSED CRUSHING INJURY TO FINGERTIP, LEADING TO PARTIAL LOSS OF DISTAL PORTION OF FINGERTIP.

Fall onto or against objects

Descending ladder - foot slipped off ladder rung, hit knee on ladder. Accident occured on 12/19/06 -did not seek medical attention until 1/24/07.

Contact with hot objects or substances

Cutting piece of metal w/cutting torch - hot slag fell onto pant leg causing burn to right calf.

Fall from ladders

Standing on second rung of ladder, slipped and fell to floor, striking elbow on floor. He fell approx 2'.

Over-exertion in pulling or pushing objects

Moving barrel of grease - while rolling & pulling on barrel received strain to right lower back.

Struck by flying object

Chipping rust off plate, a particle of rust entered his left eye behind glasses.

Knocking drag socket wedges out of drag sockets, he slipped and twisted lower back.

Fall to the walkway or working surface

Pulling on pry bar, bar slipped, employee fell to ground striking elbow on ground. Did not seek medical attention until 11-6-06. Off work since 11-7-06.

Replacing screen on Siene Bend-while replacing screen strained left lower back-worked until 9/18/07

Pulling material out of jammed jig crusher. Strain to left rib cage area.

Walking down steps, slipped and grabbed railing, pulled right arm causing torn rotator cuff to right shoulder. Also abrasion to left leg calf. Disability did not start until 2/24/06.

REPAIRING VEHICLE. LYING UNDER VEHICLE, PULLING ON WRENCH. FELT PAIN IN LOWER LEFT ABDOMEN (HERNIA). ACCIDENT OCCURRED ON 2/15/06, WENT TO DOCTOR ON 2/20/06, DISABILITY BEGAN ON 2/22/06.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was cutting shaft from the drag drum and inhaled chemical fumes.

Dismounting crane, foot slipped off bottom step, jarred back. Experienced back pain. Accident occurred 1-14-06. Pain persisted. Returned to doctor 2/17/06. Off work from 2/20/06 with disc problem.

USING PRY BAR TO ALIGN GUARD ON DRAG PINNION PUSHED ON BAR, FELT PAIN IN HIS NECK.

Fall onto or against objects

Climbing into dragline compartment, he slipped - weight of body on left arm. Did not seek medical attention until 1/4/07. Began physical therapy on 1/8/07.

Opening garage door to enter building.

Changing brushes in generator set - foreign material entered right eye.

Driving truck. Turned air conditioner fan on. Debris blew into right eye.

Removing spare parts from trailer, he felt pain. The insect bite became abcessed.
