Struck against stationary object

Stepped on pipe clamp - Twisted left foot

Fall from scaffolds, walkways, platforms

Standing on wooden plank-slipped-fell approx. 12 to 15 inches. Landed on left wrist-causing wrist fracture. Accident occured on 11/11/09. Did not seek medical attention until 11/16/09. Off since 11/16/09.

Contact with hot objects or substances

EE was cutting pipe with plasma cutter while cleaning piece of slag from tip, inadvertently compressed trigger and this caused arc to strike finger tip.

Over-exertion in lifting objects

Lifting hood on truck. Experienced pain in neck/shoulder & back area.

Fall from machine

Dismounting truck, foot slipped, landed with full weight on left foot.

Absorption of radiations, caustics, toxic and noxious substances

Cutting open a crack on fairlead of dragline. While cutting, hot oil & grease from inside the crack, shot out striking the employee in the facial area causing burns around his burning goggles.

Fall to the walkway or working surface

Tripped going up steps, fell on left arm. Accident occurred on 2/12/09, did not seek medical attention until 6/25/09.

Struck by falling object

Under haul truck changing starter. Starter slipped striking employee causing laceration requiring sutures.

Walking from picking truck to job ee-slipped on snow covered ground-fell to ground.

Diagnosed with carpal tunnel syndrome, worked until 1/21/10, off since that date.

Employee collapsed, suffering from an epileptic seizure on mine property. Employee did not notify the company of this pre-existing condition prior to or during employment. The employee has yet to return to work.
