A ROOF FALL IN THE EAST MAINS IN #8 ENTRY #9 X-CUT MEASURING APPROX 60' BY 20' BY 6-9'. THE AREAHAD BEEN SUPPRTED WITH 6' TENSION ROOF BOLTS AND 9' MEGA BOLTS AND TIMBERS. THERE WAS UNDETECTED SLIP IN THE MINE ROOF.
WHILE CUTTING THE FACE OF #8 ENTRY ON SECTION 001-0 THE CONTINUOUS MINER CUT INTO OLD MINE WORKS CAUSING AN INUNDATION OF WATER RESULTING IN THE EVACUATION OF THE ENTIRE MINE.
A ROOF FALL MEASURING APPROXIMATELY 70' X 18' X 5' IN THE 5 WEST PANEL AT #42 X-CUT #4 ENTRY 800'OUTBY 002-0. THE AREA HAD BEEN BOLTED WITH 5'FULLY GROUTED ROOF BOLTS AND THE CAUSE WAS AN UNDE TECTED SLIP IN THE MINE ROOF.
A ROOF FALL MEASURING APPROX 160' X 20' X 5' ON THE 002-0 SECTION IN THE 5 WEST , #44 X-CUT IN #4, #5 & #6 ENTRY. THE CAUSE WAS A HIDDEN SLIP IN THE MINE ROOF.
PULLING MINER CABLE WHEN HE FELT A PAIN IN HIS LOWER BACK. THIS DID NOT BECOME REPORTABLE UNTIL 11-13-00.
EMPLOYEE BENT OVER TO LOWER A PIECE OF METAL TO THE GROUND AND COULD NOT STRAIGHTEN BACK UP.
USING A DIESEL LO-TRAC TO HELP INSTALL STEEL ARCHES WHEN AN ARCH CAME LOOSE STRIKING HIM ON THE HEAD.
WALKED HEAD FIRST INTO A BENT PIN PLATE AND JAMMED HIS NECK.
EMPLOYEE WAS LOADING ROCKDUSTER FROM BULK TANK &WHILE REMOVING HOSE IT BLEW OFF STRIKING HIM IN THE MOUTH.
EE TOLD HIS FOREMAN THAT HE COULD NOT ROOF BOLT ANYMORE BECAUSE HE HURT HIS BACK. THIS WAS NOT TURNED IN TO COMPANY PERSONAL AS A MINE INJURY UNITL 11/1/00
