Accident type, without injuries

A roof fall occurred in the 1st North Sub Main in the #5(belt)entry between crosscut 29 and crosscut 30. No injuries or ventilation short circuit occurred as a result of the roof fall.

During a 6.2 inch rainfall and due to a power outage, water entered the mouth of the mine, rising to approximately 6 to 8 inches for a distance of 10 to 12 crosscuts blocking the secondary escapeway.

Employee was opening equipment doors when ee Getman tractor rolled into ee lower legs resulting in fractures to both legs.

Struck by falling object

Miner was in the process of erecting concrete block permanent stopping when rock became dislodged between roof bolts, fell, striking miner in the back and head area.

Employee was making first cut in 4 right, spad +1860 Panel 21B, #1 unit. EE was standing with legs spread due to mining height. A rock, 4' x 4' x 4 inches thick fell from between four roof bolts, hanging on the outby rib bolt's board causing it to swing down and strike the employee's left leg resulting in a lower leg fracture.

Miner repairing shuttle car cable using utility knife, when knife slipped from the cable the miner received a laceration to the left index finger requiring five (5) sutures.

Fall onto or against objects

Employee was assisting the unit mechanic with changing shuttle car tire, the tire slipped causing the employee to fall on the tire, striking the wheel rim just above EE's right eye, resulting in 5 sutures.

Caught in, under or between a moving and a stationary object

While helping plant mechanic install belt on pump, employee's left middle finger caught between the belt and pulley resulting in a slight fracture of the finger.

Employee was cutting the bands off a bundle of top rollers when EE's knife slipped cutting EE's right wrist resulting in 6 sutures.

Fall onto or against objects

While stepping out of operator's deck of battery scoop, employee tripped on the seatback and caught them self with their right hand resulting in a slight fracure.

Struck by falling object

While reaching for a roof bolt from the top of the bolter, a small piece of rock fell from the roof striking employee on the left thumb resulting in a slight fracture.
