Accident type, without injuries

A roof fall occurred in the 3rd Southwest Mains in the intake travel way at crosscut #95. The fall occurred in an intersection.

A roof fall occurred in Panel 7 at X/C #133, this is a worked out area and in an intersection in the examiners travel way.

An unplanned roof fall occurred at crosscut 165, on the 5th 54 beltline in the #11 seam. The roof fall is located in an intersection and will be cleaned up.

It was determined that the roof had deteriorated which caused the roof to fall to the mine floor. The fall was 70' long starting in the intersection of cross cut #20 along the 1st Southwest mains in the #5 entry going toward #4 entry. The fall was 18' wide and 15' plus in height.

Fall from ladders

Employee set up a 8 step ladder on unstable ground. The side he was climbing was on concrete at the other side was in loose mucky coal. Employee went to 3rd rung to check footing of ladder and ladder begin to sink. Employee jumped off ladder and broke his right ankle.

Employee was on scoop unplugging the batteries when he lost his balance and fell off the scoop. He caught his right knee in the fender well resulting in a meniscus tear.

Struck by falling object

Employee was in the process of moving an oxygen and acetylene cart when he stumbled and fell. The cart fell across the side of his right knee and leg, fracturing his lower leg.

Struck against stationary object

Driving outby, ride ran over rock, swinging ride into rib. Hit left forearm against the rib resulting in a slight fracture and surgery on 1/5/13.

Employee was in the process of moving a water hose from the 2nd floor to the bottom floor. The hose was secured to the rail with electrical tape and zip ties. While cutting the tape with a belt knife, the knife slipped cutting the corner of his left thumb, resulting in 3 sutures.

Employee was cutting belt to make a splice using a belt knife. He was cutting towards himself when the knife slipped off the belt cutting his right thigh resulting in 6 sutures.

Driving up road on the way back from 4th 54 belt drive Didn't see roof bolt and made contact causing a cut on the top of his head resulting in 5 staples.

While exiting a loader on the surface, the miner scraped his right lower leg on a fire suppression retention bracket causing a skin tear that required 18 sutures to close the wound.

Miner was hanging high voltage cable when cable slipped from his hand striking him in the mouth, causing a broken tooth.

While lifting a pipe with a chain hoist, chain slipped from pipe. The pipe struck the miner in the right side causing (2) fractured ribs.

Employee was removing rock duster hose with belt knife, Knife slipped, cutting tip of left ring finger - resulting in five sutures.

Miner was spreading legs of step ladder when ladder began to fall, miner attempted to restrain ladder and reports lower back strain/sprain. This 7000-1 form is being completed to terminate citation # 8513327. Miner reports the accident occurred on 3-22-13,however did not report accident until 5-22-13. Therefore there were 0 days lost or restricted duty.

Holding bolt on blade edge of D-9 blade while using impact wrench to remove bolt. Bolt head spun, cutting right index finger, requiring 5 sutures.

While rock dusting employee was taking hose off of duster when hose came off, it struck his left ring finger resulting in a slight fracture to his knuckle.
