Roof fall occurred at x-cut 54 between 10 & 11 entries of panel 5. The roof fall measured 50'L x 18'W x 8'T. The roof fall will not be cleaned up. The roof was supported with 5' fully grouted roof bolts. The date of development was 11/2018.
Roof fall occurred in the 9 seam in the 1st Southeast sub main entry 2 between footage markers 5220 and 5140. The roof fall measured 25'L x 20'W x 10'T. The roof fall will not be cleaned up. The roof was supported with 6' fully grouted roof bolts. The date of development was 12/2018.
FMC employees were attempting to seal the hoist shaft at the Bremen Portal at Ken American Paradise Mine No. 9. Steel beams were welded in place metal decking was secured covering the shaft. Methane accumulated in the shaft and caused a series of two explosions 4-5 seconds apart. The second explosion killed FMC employee. No other fatalities or injuries occurred.
Employee was on man cage while exiting the mine. The employee bent over to pick up lunch box and was struck on the right shoulder with a piece of ice.
Employee was operating diesel mantrip on 3B supply road and a piece of rock fell struck employee's left shoulder.
