undetected slip in roof, Rock fell measuring 0-3' feet wide, 0-30"" high and 10' long at crosscut 203, #5 entry in the #11 seam.
A roof fall occurred in the #9 seam in the 1st West Sub-Main at crosscut #7 starting at edge of the pillar line between the #2 and #3 entries and extending half way into the pillar line between #3 and #4 entries. The fall measured approximately 80'x18'x10'. The area was supported with 6' tension rebar roof bolts and 8' double lock mega bolts.
A roof fall occurred in the #11 seam in the 2 Main North between crosscut 98 and 99 in the #4 entry. The fall measured approximately 50'x8'x5'. The area was supported with 30"" fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

The fall was timbered out with cribs built in intersections connected to area.

Shuttle Car operator attempting to retrieve sideboard from shuttle car that had fallen into car when he felt a burning sensation in his abdomen, resulting in a hernia.

Employee tripped over pipe, sprained right foot.
The employee picked up a belt roller and when he turned to install it he fell to the ground and was having pain in his lower back and left hip. The employee was not taken off of work by a doctor until 10/10/2011.
Employee was walking up the beltline when right knee gave out and he felt a sharp pain in right side of knee. Worked regular duty until he had knee surgery on 1-18-12, becoming a lost-time accident.
Employee was sitting on a man trip when he was pulled out of the man trip by a mine employee fracturing his right ankle.
Employee was in 8 right on #3 unit loading coal when he bumped his right ear on a pin plate. The ear was cut requiring 4 stiches.
Walking near power center, struck head and left ear on roof bolt bearing plate, causing laceration to left ear requiring 7 sutures.
While operating a battery scoop, there were some chains in the operators compartment and one of the chain ends fell through a hole in the operators compartment and was run over by the wheel and as the chain jerked the hook end struck the employee on the index finger of the left hand. He received 6 sutures.
While installing roof bolts the employee had just drilled the first part of a two part hole and as he lowered his boom and reached for the second drill steel, the first drill steel came out of the hole unexpectedly and struck him on the top of the left thumb fracturing it.
Employee was installing roof bolts and as he was kneeling on some rock his knee slipped and as he reached to catch himself with his left hand he hit the boom lift lever and as the boom lifted, his right hand was on the boom and his index finger was mashed between a bolt on the boom and the boom itself. He received 6 sutures.

While loading supplies into a battery scoop bucket, a metal band hit the employee on the right hand causing a laceration. The employee received 4 sutures.
While changing a cat pad on the continuous miner the employee smashed his thumb between the pan and a board.
While installing roof bolts, the employee swung his boom out next to the rib and a piece of rock fell from the mine roof striking him on the back and foot.
While using a hammer to hit another hammer, a sliver of metal from one of the hammers struck and embedded in the employee's left forearm.
The employee had his hand on the tail of the continuous miner when a piece of rock fell from the mine roof and mashed his ring finger on the left hand. The employee received 5 sutures.
While loosening a nut with a wrench, the nut broke loose suddenly and the employee hit his left thumb against a sharp piece of metal causing a laceration. The employee received 9 sutures.
Lifting wide beam. Mashed hand between beam and 630 mucker, causing crushing injury to right little finger.
Leveling off muck pile to install panning cycle. Checking grade, rock fell hit left shoulder causing employee's knee to hit mucker. Shoulder strain and bruises to left knee.
While changing the female receptacle on a battery scoop the allen wrench slipped off and came into contact with the positive terminal and an arc occurred causing the employee to receive 2nd degree burns on his left hand/thumb/wrist.
While splicing a continuous miner cable, the utility knife the employee was using slipped and cut him on the right thumb causing him to receive sutures.
While unloading some steel pipes, one of the pipes shifted unexpectedly and pinched the employee's left wrist between two of the pipes. The employee received a hairline fracture.
While prying on a cat pad, the pry bar slipped and the employee hit the back of his hand on a jagged piece of metal causing a laceration. The employee received 1 suture to the middle finger on his left hand.
While operating a roof bolting machine the drill steel became stopped up and when the employee dropped the boom to check the problem he positioned his hand on top of the boom and the drill steel fell out of the hole and struck him on the finger. The employee received sutures.
