This area was supported with 30 inch fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

Employee had been under a doctors care on his right shoulder since May 2007 for repetitive shoveling. He re-aggrevated the shoulder on 12/4/2007 while shoveling on a belt tail roller. He went off work for shoulder surgery on 2/13/2008.
Employee stepped on a piece of coal while putting a box of glue on top of a roof bolter, causing a strain/sprain to the right knee. This did not become a Lost Time Accident until 10/31/2007.
Employee stated: that he picked up a box of pinner glue, turned around falling to the ground twisting the right knee. This injury did not become a lost time until 9/24/07.
Employee was operating a diesel tractor and struck his head on an overcast. He sustained a teardrop fracture to one of the vertebra in his neck.
Employee was hanging a piece of mine curtain in the face of #10 entry on #2 unit when a small piece of rock fell from the mine roof striking his right shoulder. This did not become a Lost Time Accident until September 6, 2007.
Employee was building an overcast and as he was lifting material for the overcast he strained his left shoulder. This did not become a Lost Time Accident until 8/13/2007.
While employee was getting off of a personnel carrier he hit his head on a beam and jammed his neck. Employee continued to have trouble for about 3 months and eventually had to have surgery. This did not become lost time until 9-17-07.
Employee was operating a scoop and ran over a piece of rock that was on the roadway, causing him to strain his lower back. This did not become a Lost Time Accident until 6/1/2007.
While installing supplemental roof support a piece of rock fell from the mine roof striking employee on the head, left hand and lower back. Employee received 5 staples in the head and 3 sutures on the left hand.
Employee was lifting up on one side of a pinner pot for a trussbolter and strained his groin area. This became a Lost Time Accident on March 28, 2007.
While tramming a Getman Tractor on the supply road employee hit a pot hole and injured his back. This incident did not become lost time until 11/05/2007.
A diesel scoop was traveling inby, at the same time a 3-man tractor was pointed outby and parked against the left rib. As the scoop passed the tractor, the bucket of the scoop hit the right leg and left foot of the tractor operator. The tractor operator sustained 4 broken bones to the left foot and a long laceration on the inside of the left foot.
A piece of rock fell from the mine roof, striking the employee on the left hand and thigh of the left leg. The employee received 4 sutures to the left hand.
While cutting a suction hose for a roof bolter, employee cut his index finger on his left hand. Employee received 3 sutures.
Employee stated: While walking up the slope he hit his right hand on i-beam, resulting in his ring finger being cut.
Employee was changing a shuttle car tire out and mashed the pinky finger on the right hand between the shuttle car and shuttle car tire causing a laceration to the finger. He received four (4) sutures.
Employee was using a pair of cutters to cut the metal bands on a bundle of nine (9) foot roof bolts and one of the metal bands cut his left wrist causing a small laceration. He received five (5) sutures.
Employee was installing a solid belt roller, when the roller slipped, striking his index finger on the right hand causing a laceration. He received 5 sutures.
Employee was removing a metal pin, to uncouple a rock duster from a supply tractor, when his index finger on his right hand was struck by the metal pin. He received 2 sutures.
Employee was roof bolting and was struck in the upper lip by a small piece of an unidentified foreign object causing a small laceration. He received 4 sutures.
Employee struck his left elbow against a roof bolt plate causing a laceration to the elbow. He received 3 sutures.
