A roof fall occurred in the #9 seam in the 7 East Panel at crosscut #14 in the intersection of #7 entry extending inby and outby for 25 feet measuring approximately 60ft. x 18ft. x 9ft. high. This area was supported with 5 foot fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occurred in the #11 seam in the 2 Main North cross-cut #172 in the intersection of #2 entry and extending through the intersection of #3 entry, measuring approximately 80ft. x 18ft. x 4ft. to 5ft. high. This area was supported with 30 inch fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occured in the 3 south panel in the #2 entry in the intersection of crosscut #13. The fall measured approximately 30'x18'x6'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.

A roof fall occurred in the #9 seam in the 2 North Panel off the 2 Main East in #10 entry between crosscut #39 and crosscut #40 measuring 30 ft. x 18 ft. x 5 ft. high. This area was supported with 5 ft. fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occurred in the #11 seam in the 2 East in the intersection of crosscut #5 in #7 entry extending through the pillar line approximately 15 feet towards #6 entry meauring approximately 50ft. x 18ft. x 6ft. high. This area was supported with thirty (30) inch fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occurred in the #9 seam in the Main North at crosscut #21 in 1/2 the intersection in #3 entry measuring approximately 18ft. x 10ft. x 6ft. high. The area was supported with 6t. tension rebar bolts and 12 ft. cable bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occurred in the 1 North Panel at crosscut #6 in the intersection of #7 entry and extending inby and outby both pillarlines 20ft. measuring approximately 58ft. x 19ft. x 7ft. high. The area was supported with 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occurred in the #9 seam in the 1 Main West at crosscut #38 in the intersection of #4 entry and continuing through the pillarline toward #5 entry measuring approximately 50ft. x 19ft. x 7ft. high. The area was supported with 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

A roof fall occured in the 2 main north, crosscut #47, in the pillar line between #5 and #6 entries and then at 10:45 pm it traveled through the intersection of the #5 belt entry. The fall measured approximately 40'x19'x6'. The area was supported with 6' tension rebar bolts and 6""x6"" square timbers.
A roof fall occurred in the 3 south panel, in the last open crosscut, approximately 15' on either side of the intersection of #6 entry. The fall measured approximately 50'x18'x6'-9'. The area was supported with 5' fully grouted bolts and 9' mega bolts.

The area was supported with 5 foot fully grouted roof bolts. An undetected slip was determined to be the cause of the fall. This area was mined in May 2007.
A roof fall occurred in the 1 Main East in the intersection of #4 entry at cross-cut #12 and extended part of the way through the pillar line between #4 entry and #5 entry measuring approximately 40ft. x 19ft. x 6ft. The area was supported with 6 foot tension rebar bolts. An undetected slip was determined to be the cause of the fall. This area was mined in February 2004.
A roof fall occured in the 1 Main West, #6 entry, starting mid pillar line between crosscut #38 and #39 and ending at the outby edge of crosscut #40. The fall measured approximately 66'x19'x9'. The area was supported with 5' fully grouted bolts and 9' mega bolts.
A roof fall occured in the 1 main east, crosscut #111, starting in the right pillar line of the #4 entry and ending in the left pillar line of the #6 entry. The fall measured approximately 90'x19'x6'-7'. The area was supported with 6' tension rebar bolts and 9' double lock mega bolts.
A roof fall occured in the 2 Main North, #1 entry, in the pillarline between crosscuts #20 and #21. The fall measured approximately 50'x19'x6'. The area was supported with 6' tension rebar bolts and 6""x6"" timbers.

A roof fall occured on the MMU 001-0 in the 3 south panel, #6 entry, in the intersection of crosscut #22. The fall measured approximately 30'x15'x5'. The areas was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occured in the 3 south panel, #9 entry, starting halfway between crosscut #11 and #12 and ending halfway between crosscuts #12 and #13. The fall measured approximately 75'x18'x9'. The area was supported with 5' fully grouted bolts and 9' double lock mega bolts. An undetected slip was determined to be the cause of the fall.

The area was supported with 6ft. tension rebar bolts and 9ft. mega bolts. An undetected slip was determined to be the cause of the fall. This area was mined in April 2004.

A roof fall occurred in the 4 South Panel #9 entry in the pillarline between crosscut #13 and crosscut #14 measuring approximately 35ft. x 19ft. x 5ft. The area was supported by 5 foot fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
A roof fall occurred in the #11 seam #7 entry in the pillarline inby cross-cut #34 and extending through the pillarline between cross-cut #35 and cross-cut #36 measuring approximately 115ft. x 18.5ft. x 4.5ft - 5ft. The area was supported by 30 inch fully grouted roof bolts. A slip was determined to be the cause of the fall.
A roof fall occured in the 6 south panel, #6 entry, starting at crosscut #24 and ending at crosscut #25. The fall measured approximately 60'x 19'x 5'-9'. The area was supported with 5' fully grouted bolts and the intersections have 5 additional 9' mega bolts. An undetected slip was determined to be the cause of the fall.
Employee had been under a doctors care on his right shoulder since May 2007 for repetitive shoveling. He re-aggrevated the shoulder on 12/4/2007 while shoveling on a belt tail roller. He went off work for shoulder surgery on 2/13/2008.
Employee stepped on a piece of coal while putting a box of glue on top of a roof bolter, causing a strain/sprain to the right knee. This did not become a Lost Time Accident until 10/31/2007.
Employee stated: that he picked up a box of pinner glue, turned around falling to the ground twisting the right knee. This injury did not become a lost time until 9/24/07.
Employee was operating a diesel tractor and struck his head on an overcast. He sustained a teardrop fracture to one of the vertebra in his neck.
Employee was hanging a piece of mine curtain in the face of #10 entry on #2 unit when a small piece of rock fell from the mine roof striking his right shoulder. This did not become a Lost Time Accident until September 6, 2007.
Employee was building an overcast and as he was lifting material for the overcast he strained his left shoulder. This did not become a Lost Time Accident until 8/13/2007.
While employee was getting off of a personnel carrier he hit his head on a beam and jammed his neck. Employee continued to have trouble for about 3 months and eventually had to have surgery. This did not become lost time until 9-17-07.
Employee was operating a scoop and ran over a piece of rock that was on the roadway, causing him to strain his lower back. This did not become a Lost Time Accident until 6/1/2007.
While installing supplemental roof support a piece of rock fell from the mine roof striking employee on the head, left hand and lower back. Employee received 5 staples in the head and 3 sutures on the left hand.
Employee was lifting up on one side of a pinner pot for a trussbolter and strained his groin area. This became a Lost Time Accident on March 28, 2007.
While tramming a Getman Tractor on the supply road employee hit a pot hole and injured his back. This incident did not become lost time until 11/05/2007.
A diesel scoop was traveling inby, at the same time a 3-man tractor was pointed outby and parked against the left rib. As the scoop passed the tractor, the bucket of the scoop hit the right leg and left foot of the tractor operator. The tractor operator sustained 4 broken bones to the left foot and a long laceration on the inside of the left foot.
A piece of rock fell from the mine roof, striking the employee on the left hand and thigh of the left leg. The employee received 4 sutures to the left hand.
While cutting a suction hose for a roof bolter, employee cut his index finger on his left hand. Employee received 3 sutures.
Employee stated: While walking up the slope he hit his right hand on i-beam, resulting in his ring finger being cut.
Employee was changing a shuttle car tire out and mashed the pinky finger on the right hand between the shuttle car and shuttle car tire causing a laceration to the finger. He received four (4) sutures.
Employee was using a pair of cutters to cut the metal bands on a bundle of nine (9) foot roof bolts and one of the metal bands cut his left wrist causing a small laceration. He received five (5) sutures.
Employee was installing a solid belt roller, when the roller slipped, striking his index finger on the right hand causing a laceration. He received 5 sutures.
Employee was removing a metal pin, to uncouple a rock duster from a supply tractor, when his index finger on his right hand was struck by the metal pin. He received 2 sutures.
Employee was roof bolting and was struck in the upper lip by a small piece of an unidentified foreign object causing a small laceration. He received 4 sutures.
Employee struck his left elbow against a roof bolt plate causing a laceration to the elbow. He received 3 sutures.
