A rockfall occured in the Main North at crosscut #9 in the intersection of #6 entry. The fall measured 20'X20'X6'. This area was mined in September of 1996 and was supported with 6' tension rebar bolts. The fall was caused by an undetected slip.

A roof fall occured in the South West Vent Entries at crosscut #18 in the #6 and #7 entries. The fall measured 70' X 20' X 6'. This area was mined in August of 2000 and was supported with 6' tension rebar bolts and timbers. The fall was caused by an undetected slip.

The fall measured approximately 115' X 19' X 5'. The area was supported with 5' fully grouted rebar bolts. The fall was caused by an undetected slip.

THE FALL MEASURED APPROXIMATELY 110' X 19' X 5'. THE AREA WAS SUPPORTED WITH 5' FULLY GROUTED REBAR BOLTS. THE FALL WAS CAUSED BY AN UNDETECTED SLIP.

Over-exertion (Not Elsewhere Classified)

Employee was loading roof bolt supplies onto a roof bolter. He twisted his left knee while loading a box of glue. This did not become a Lost Time Accident until April 27, 2005.

Fall onto or against objects

Employee had his left leg on top of a battery scoop while inserting the plug of a battery powered rockduster into the scoops PTO. His leg slipped striking it against the battery scoop causing a contusion to the shin.

Over-exertion (Not Elsewhere Classified)

Employee was on his knees shoveling under the boom of a belt head drive. When he stood up he twisted his right knee causing a medial menisus tear. This did not become a lost time accident until 10/26/2004.

Struck by rolling or sliding object

Employee was installing the second roof bolt in a row of bolts when a piece of rock fell from the mine roof and hit the ATRS then rolled under the ATRS striking him in the left leg on the upper thigh.

Struck by flying object

Employee was advancing the power on the #2 section. As he was dropping the continuous mining cable, small fine rock and rockdust fell from the mine roof covering his head and the top part of his body. As he was brushing the material from his head and top part of his body some of the loose material entered both of his eyes. He sustained a scratch to the cornea of his right eye.

Over-exertion (Not Elsewhere Classified)

Employee is a roof bolter operator. He had drilled a hole in the mine top to install a roof bolt. After he inserted the roof bolt he was attempting to straighten the bolt out to finish the installation process by pushing on the roof bolt. This caused him to loose his balance causing his left knee to twist. (This did not become reportable until 07/26/04.

EMPLOYEE WAS CRUSHED BETWEEN A COAL RIB AND A BATTERY POWERED ROCKDUSTER BEING PULLED BY A BATTERY POWERED SCOOP RESULTING IN A BROKEN LEFT SHOULDER BLADE AND BROKEN LEFT ARM. HE ALSO RECEIVED MULTIPLE CONTUSIONS.

Fall to the walkway or working surface

WALIKING ON RETURN SIDE OF A3 BELT HEADER AND SLIPPED ON A MUDDY SURFACE CAUSING HIM TO FALL ON HIS LEFT SIDE.

Struck by falling object

EE WAS WORKING ON THE INSTALLATION OF A NEW BELT SET-UP ON #1 UNIT. HE BENT OVER TO PICK UP A BELT ROPE OFF THE GROUND WHEN A PIECE OF ROCK 4' X 5' X 3"" THICK FELL FROM THE MINE ROOF AND STRU CK HIM ACROSS THE BACK AND LEFT KNEE. BECAME A LOST TIME 3/3/04. THE PIECE OF ROCK FELL OUT BETWEEN PINS.

EMPLOYEE WAS TIGHTENING ROPES ON #1 UNIT TAILPIECE AND SLIPPED ON WET GOB TWISTING HIS RT. KNEE. (THIS DID NOT BECOME REPORTABLE UNTIL 4/12/04).

Struck by falling object

EMPLOYEE WAS RIDING IN A 4-MAN DIESEL RIDE WHEN A PIECE OF ROCK, MEASURING 3'X 6'X 1-2""THICK, FELL FROM BETWEEN ROOF BOLTS STRIKING HIM IN THE HEAD, SHOULDER & RT. LEG.

Over-exertion in lifting objects

EE WAS MOVING A PIECE OF ROCK THAT HAD FELL FROM BETWEEN ROOF BOLTS AT HIS CAR ANCHOR WHEN HE FELT PAIN IN HIS LOWER BACK. HIS FIRST DAY AWAY FROM WORK WAS 1-12-04.

EE WAS STANDING IN #5 ENTRY ON #1 UNIT WITH HIS RIGHT ARM RESTING ON TOP OF HIS CANOPY WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF BOLTS STRIKING HIM ON THE ARM & HAND.

Employee was reclaiming belt structure when a piece of structure pinched the palm of his right hand causing a laceration. He received 10 sutures.

Struck by... (Not Elsewhere Classified)

Employee was using an axe to break the metal bands that hold brattice block together and one of the bands, after being struck, recoiled back striking the employee on the right side of his chin causing a leceration. The laceration required 4 sutures by a physician.

Caught in, under or between a moving and a stationary object

Employee was tramming a battery scoop on the #2 unit and he had his left hand through the hand hold, on the canopy, that is used to assist someone getting in and out of the scoop. When he turned toward a cross-cut his hand became caught between the mine roof and the top of the scoop canopy resulting in lacerations to his index and middle finger. He received a total of 7 sutures.

Struck by flying object

Employee was attempting to break apart a feeder conveyor chain by striking it with a hammer when a piece of the hammer broke off striking him on the chin. The piece of metal lodged up under the skin requiring it to be removed by a physician. He received 4 sutures.

Struck against stationary object

The employee was using a high pressure washer to wash a refuse truck. The pressure hose burst into at the wand causing his right hand,that was holding the wand, to travel backwards striking the headlight of the refuse truck causing lacerations to the back of his hand and pinky finger requiring him to receive 14 sutures.

Struck against stationary object

Employee was hitting on a plastic (pump line) pipe, with a hammer, attempting to insert one section of pipe into another section of pipe when a piece of the pipe he was hammering on broke causing his left hand to strike the pipe cutting his index finger. He received 4 sutures.

Caught in, under or between a moving and a stationary object

INSTALLING 2"" COATED STEEL CONDUIT, HE GOT HIS FINGER BETWEEN THE CONDUIT AND A CONCRETE WALL, MASHING HIS FINGER, REQUIRING TWO STITCHES.

Struck against stationary object

Employee was re-claiming belt line control cable in the 1 North Panel cross-cut 80 in #5 entry. He was pulling on the belt line control cable and hit his right arm on a roof bolt plate causing it to be cut right above the elbow. He received 4 sutures.

Struck by flying object

ROOF BOLTING AT THE FACE & WAS GOING TO TAKE THE SECTIONAL DRILL STEELS OUT WHEN THE TOP & BOTTOM SECTION GOT STRUCK TOGETHER. WHEN HE SHOOK THEM TO BREAK THEM APART THE BOTTOM SECTION OF THE STEELS STRUCK HIM IN THE MOUTH.

INJURED WAS STANDING ON GOB AND DEBRIS WHILE LOADING CRIBS, LOST BALANCE AND SLIPPED AND FELL BREAKING HIS RIGHT ANKLE.
