A roof fall occurred in the #9 seam in the 1 North at crosscut 107 in the pillarline between #6 entry and #7 entry measuring approximately 20ft. x 19ft. x 4.5ft. high. This area was supported with 5ft. fully grouted roof bolts.

A roof fall occurred in the #9 seam in the East Mains in the #6 entry in the intersection of crosscut #24. The fall measured approximately 20'x 20'x 9'. The area was supported with 6' tension rebar bolts, 9' mega bolts and truss bolts.

A blockage or roof rock occurred in the #9 seam in the East Main at crosscut #17 causing an area of unsupported roof. The roof rock measured approximately 12ft. x 12ft. x 4ft. high. The blockage was in the #6 entry and is the secondary escapeway. This area was supported with 9 foot mega bolts.

A roof fall occurred in the #9 seam in the 2 Main East, #9 entry, 1/2 the intersection of xc 19 and extending through the pillar line towards xc 20 and from 1/2 the intersection of xc 19 through the pillar line towards #10 entry. The fall measured approximately 60' x 18'x 6'-9'. An undetected slip was determined to be the cause of the fall.

A roof fall occurred in the #9 seam in the 1 North at crosscut #70 in the pillarline between #6 and #7 entries measuring 35ft. x 18ft. x 5ft.-6ft. high. This area was supported with 5 foot fully grouted roof bolts.

The fall measured approximately 80'x18'x7'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.

The area was supported with 5ft. fully grouted roof bolts and bacon straps. A slip was determined to be the cause of the fall.

A roof fall occurred in the 1 South, crosscut #18 in part of the intersection of #5 entry and extending inby toward the face and covered the back 10' of a roof bolter. The fall measured approximately 35' to 40'x3'to 19'x7' to 8'. The area was supported with 3' fully grouted bolts and 5' fully grouted bolts. A slip was determined to be the cause of the fall.

A roof fall occured in the 7 East panel crosscut #15, starting in the #9 entry and proceeding through the #10 entry. The fall measured approx. 80'x18'x6x. The area was supported with 5' fully grouted roof bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.

The employee had wrapped a thin cable around the brake handle on a piece of equipment and was going to pull the cable but, while he was trying to hang onto the cable it caught on something and pulled tight and caught his left hand and broke his middle finger. The nature and location of the injury caused the employee's finger to be amputated by the doctor.

Struck against a moving object

Employee was riding in a mantrip and strained his back when the mantrip bumped a coal rib as it was being turned around.

Employee was in the process of making a splice in a piece of mine belt. As he was cutting the belt with a utility knife the knife slipped causing a small laceration to the left knee. He received 4 sutures.

A piece of rock fell from the mine roof striking the employee on the right foot.

Employee was roof bolting, in a high place that was cut out for a Header Hole, when a piece of rock fell from the mine roof striking his left shoulder. This did not become a Lost Time Accident until 9/9/08.

Over-exertion (Not Elsewhere Classified)

Employee was walking when he stepped in a hole, in the mine floor, causing a strain/sprain to his left leg and knee. This did not become a Lost Time Accident until 8/22/2008.

Over-exertion in pulling or pushing objects

Employee was dragging loops of cable of a continuous miner around the corner of a coal rib and strained his back.

Caught in, under or between a moving and a stationary object

While being assisted by a 2-man ride down the #9 seam slope, the golf cart lost its brake and the front of the golf cart struck the right side of the slope. The 2-man ride stopped and the golf cart turned slightly sideways, slamming into the back of the 2-man ride pinning the employee in the golf cart seat causing a laceration, fractured pelvis and two fractures to the vertebra.

Over-exertion (Not Elsewhere Classified)

Employee was stepping out of the operator's compartment of a diesel scoop when his right foot slipped causing pain in his left leg. Upon completion of medical tests it was determined his lower back was initially injured and was the cause of the pain in his left leg. This did not become a Lost Time Accident until 6/6/2008.

Over-exertion (Not Elsewhere Classified)

Employee was pre-shifting on #4 unit in the face of the #3 entry when he stepped in a hole, in the mine bottom, causing him to strain his left knee.

Struck against a moving object

Employee was operating a roof bolter and strained his lower back. This did not become a Lost Time Accident until 5/28/2008.

Over-exertion (Not Elsewhere Classified)

Employee was walking toward the bottom of the Draw off tunnel when he slipped on a small piece of ice causing him to hyperextend his left knee causing a strain/sprain to the knee. This did not become a Lost Time Accident until 5/5/08.

Fall to the walkway or working surface

Employee was walking from the office trailer to the warehouse when he slipped and fell on the pavement injuring his left shoulder. This did not become a Lost Time Accident until 4/16/2008.

Struck by powered moving object

While hanging a line curtain in the #1 entry the employee was slightly bumped with the battery end of a battery scoop causing a contusion to the right leg and hip.

Over-exertion in lifting objects

Employee was picking up a metal trailer hitch (tongue) off of the ground in the supply yard and strained his back.

Fall from machine

Employee was walking on the #3 unit and stumbled causing him to fall to the ground injuring his right foot.

Over-exertion (Not Elsewhere Classified)

Employee was examining a belt line when he stepped on the 4"" waterline causing him to slip straining his right knee. This did not become a Lost Time Accident until 11/10/2008.

Fall from machine

Employee was getting off a Getman Tractor when he slipped causing him to fall on his left side causing a strain/sprain to his left shoulder. This did not become a Lost Time Accident until 2/13/2009.

Struck by falling object

Employee was operating a single boom bolter installing 12 foot cable bolts. He was using a 3ft. wrench to tighten the bolt. When he let the boom down the wrench fell striking his left ear causing a laceration. He received 3 sutures.

Struck against stationary object

Employee was walking across the working section and ran into a roof bolt plate, that was on a brow, causing a laceration above his left eye. He received 4 sutures.

While opening a glue box with a utility knife, the knife slipped and cut the employee on the left wrist. The employee received 4 sutures.

Struck by falling object

Employee laid his left hand on top of a 4 man diesel ride when a piece of rock fell from the mine roof striking his hand causing a fracture and laceration to the middle finger. The finger received 5 sutures.

Employee was cutting a piece of rubber off a tire of a supply trailer with a utility knife when the knife slipped causing a laceration to his right arm. He received 6 sutures.

Struck by falling object

A small piece of rock fell from the mine roof striking the employee on the back of the right hand causing a laceration. He received 12 sutures.

Struck by falling object

Working as bolter helper, was making up bolts when 4 inch rock fell, striking employees left index finger from nail to first knuckle.

Fall onto or against objects

While tightening a conveyor chain on a shuttle car the socket slipped off of the nut and employee fell face first into the bumper on the shuttle car. Employee busted his bottom lip and received 7 sutures.

Employee was helping other miners lift a continuous miner cable when he noticed pain in his lower back. Upon examination from a medical facility it was determined the pain was from degenerative disk disease. His first day away from work was 7/15/2008.
