Over-exertion in lifting objects

The SIC employee bent over to pick up a aluminum frame from the bottom and jerked up on the frame to lift it on the saw horse when ee felt a sharp pain in lower back and down leg.

Employee was pulling on a pipe wrench and strained muscles in his right shoulder.

Employee slipped in muddy conditions and fractured left knee.

Employee strained neck/shoulder muscles while pulling chain to switch diverter plate in conveyor chute.

Struck against stationary object

Employee was walking on top of rail car and stepped on latch which strained metatarsal part of foot. Employee went to doctor and was written work restrictions.

Employee was performing screen change on #8 Rotex in Coloring Building and using wrench to tighten jackscrew. The wrench slipped off the jackscrew causing strain in left shoulder.

Individual was using a 4.5 inch hand held grinder with wire wheel brush to clean mortar off of a hand trowel. Wire wheel brush caught corner of hand trowel and broke a piece off. Broken piece struck individual in back of hand causing laceration to finger.

Struck against stationary object

Employee was tightening a bolt and rubbed the back of his hand on sharp area cutting the back of left hand, 27 stitches.

Struck by flying object

Employee stated they bumped conveyor hood which dislodged dust/material which got into employee's eye. Employee flushed eye and still felt like something in it. Went to ER as precaution flushed eye again and was given prescription drops.

After completing a clean up task with the guzzler truck, operator was disconnecting 6"" hoses from a coupling. The hose he was pulling, bounced back, hitting him in the mouth. Operator was taken to the clinic for medical evaluation.

Struck by falling object

Employee was working on un-sticking a air actuated flop gate. He rested his hand under the flop gate, knelt down to investigate. The flop gate dropped unexpectedly catching his right hand ring finger, smashing the end of the finger.

Fall to the walkway or working surface

Employee reported slipping on ice and falling outside in shipping's railroad yard. Strained low back has now started chiropractic treatment.

Long term employee had left hand carpel tunnel release.

Absorption of radiations, caustics, toxic and noxious substances

Employee developed an allergic reaction at work while working in coloring. Skin turned red no blisters. Employee feels it was from working with red pigment. Employee went to doctor and they wrote restrictions to stay out of area until redness goes away.
