Struck by... (Not Elsewhere Classified)

While employee was conducting a screen change employee came in contact with sharp edge of screen resulting in a laceration that required 20 stitches

While employee was un-choking a chute; the employee had a charged air hose slip from their hand, striking them above the left eye brow splitting the skin. Employee was treated in E.R.; four stitches and two RX's.

Over-exertion (Not Elsewhere Classified)

While grabbing and cutting bags at Mixer station started to feel some pain and numbness in both hands and arms around Mid April. Employee started to stretch and treat self but symptoms have not gone away. Went to doctor and was written off for two days. Is back to full duty now.

Fall to the walkway or working surface

Employee climbed up the ladder and proceeded to set the nip on the crusher. The employee then began descent down the ladder, when they stepped off the ladder to the floor, right ankle turned causing the Employee to fall to the floor. Employee went to the control room where the shift lead called 911. Employee had a lot of pain in the upper leg/hip area.

Fall onto or against objects

Employee was walking up to the front entrance of the admin. building. Employee missed the second step. Employee fell forward; reaching out with right arm to break fall. The fall resulted in a broken upper arm. Surgery will be required as result.

Struck by falling object

Employee was walking back to a Dump Truck and a tree 12 inch diameter fell on employee fracturing the fibula lower leg and a thoracic vertebra. Employee is able to move all extremities. Spinal cord is not damaged.

Over-exertion (Not Elsewhere Classified)

Department Helper cleaning south basement with bobcat, while exiting the equipment stepping down felt a sharp pain on left foot left side of heel.

Over-exertion in lifting objects

Mixer operator irritated lower back while lifting liquid pigment into mixing pot.

Absorption of radiations, caustics, toxic and noxious substances

Employee complained of chest burning after opening bottom doors on railcars. The railcar company delivered rail cars on site that were not completely empty. Rail car contained limestone.

Struck by flying object

Employee was attempting to extract a bolt to remove a paddle from a pug mill shaft. The employee was driving out the bolt with a 3lb sledge hammer and drift pin when the employee struck the edge of the paddle causing a section to chip away and consequently flew up striking the employee in the mouth.

Emp was training another emp on the guzzler. Activating the hydraulics on the equipment causes a burst of air toward the ground. There was dust on the ground below the guzzler which appears to have been stirred up and some got into the employee's eye.

Employee was aligning a hole with an alignment pin while work partner was tightening a 1/2"" bolt with an adjustable wrench. During the tightening process, the adjustable wrench slipped off the bolt and hit the work partner in the lip and lower nose area causing a laceration to the lip and lower nose area.

Struck against stationary object

Maintenance employee was using a forklift to align a brass bushing in a socket for the 4 1/4 crusher. EE stated that EE's hard hat had fallen off several times. EE leaned over and struck head on one of the forks. EE was not wearing the hard hat at the time. This resulted in an injury requiring four staples. The employee was treated and returned to full duty the same day.

Struck by flying object

Employee was using the clothes cleaning booth. While the air system was in operation a piece of debris went around the employee's safety glasses and went into eye.

Employee wiped off face while wearing gloves. Gloves had residual metal on them from work. A sliver of metal got into the employee's eye.

Struck by rolling or sliding object

EE was performing maintenance work on aerial lift. EE was working in a tight area underneath vehicle using a hammer to tighten hydraulic hoses. Hammer slipped & slid about 18"" down the hose, hitting EE in mouth. EE reported damage to crown on tooth. EE was sent to dentist for evaluation & damage to tooth reported. Employer did not receive knowledge of damage to tooth until 3/5/19.
