Wrench slipped off the hex nut. When the wrench slipped, the box end of the wrench made contact with the base of the separator. The employee's hand slipped, from the orignal grip position, down the shaft of the wrench at an angle. This resulted in a hyper extension of the thumb on the Employee's left hand.
Mechanic caught foot and tripped on floor protectors located in the quarry garage. As employee turned to leave shop his foot caught the edge of the belting and caused him to fall onto the rubber. Employee's right forearm was sore as a result. Did not go to doctor immediatly
Employee stated that he was experiencing pain in the right and left elbow due to the repetive work that he had been doing.
Miner felt a pull in the groin area, reported to the site nurse who in turn sent him to the Occupational Health Clinic. The Clinic evaluated the Miner and diagnosed him with a left groin strain.
Employees were changing a screen on 20 rotex, old screen had been removed and new screen was going in. When employees lifted the screen to the rotex opening one employee said ""felt a pop like cracking your knuckles in mid-back area"". Employee continued working and finished screen change. Employee said it didn't hurt until walking down the stairs to bin tender floor.
Maintenance mechanic attempting to lift a crusher roll bearing end cap for repositioning on a pallet. Mechanic dropped the end cap and crushed finger between the part and the wooden pallet.
During normal work tasks employee strained low right back. The 2 tasks that contributed to the injury were; 1)Scraping snow balls off of stacker hopper bars while reaching out from catwalk. 2 )While cleaning out catwalk around 10 conv. tightner employee was shoveling and was prying a frozen chunk of mud and rock and felt a significant sharp pain in right side of low back.
Employee was walking down A row and caught a loose lid with his foot. This caused the employee to fall, striking the edge of the opening. Lacerating the left shin.
Employee strained his back when wind blew door of grader into him. Went to doctor was prescribed medication.
Employee was using a cutting rig and had a hot piece of metal fall into his left ear canal.
Employee had a 25 db STS shift change in their hearing in both ears.
Employee discovered face down in guard shack at 09:55 PM by relieving coworker.
