Accident type, without injuries

Fire started in dust collection duct work off of line 3 in our coloring dept. Fire lasted about 40 minutes. Fire Dept called. No injuries

Fall from machine

Using the Grader to plow snow from the quarry haul road. He felt a loss of hydraulic pressure in the machine. The operator stopped the machine immediately to investigate issue. As he stood up in the cab and turned around the employee stated his foot got hung up on the cab door threshold causing him to fall out of the machine onto the ground (landing on knees/legs).

Employee alledges back pain.

Employee pulled his torch hose from under a bushing and strained his back.

Fall to the walkway or working surface

As the Miner was preparing to load a truck he went outside of his office to instruct the driver and explain he would be loaded next. As he returned to his office he was rushing to start loading the truck. He fell and twisted his ankle. First Aid was applied, ice on and off for 40 minutes, he was than taken to the clinic where x-rays were taken and his ankle proved to be broken.

Miner was attempting to remove the lid/cap on the #7 mandrel. When he started to turn the cap his hand slipped off of where he was holding/pushing and he hit his hand on the left side of the pinky knuckle on the steel cap causing his hand to bruise.

Employee was shoveling up and hit a metal plate that was under a pile. Strained right wrist and arm. Was fine at time but months later employeee went to doctor and got prescription meds.

The employee was opening the doors on the 35 ton dust bin. After knocking the bottom bolt out of the door slot with a hammer, employee raised his hand to remove the top bolt. The door unexpectedly sprang open and struck the little finger of the right hand.

Employee stated walking back & forth on the Bintender floor over crossover felt something pull in left knee while descending down steps. EE said she would wait & speak with EH&S. 6/25/09, on 6/25 at noon EE said it was sore but not as bad as it had been the night before. She did feel some binding and shooting pains. EE iced & elevated the knee, stated that swelling had gone down.

Over-exertion in wielding or throwing objects

Employee had a sore back for about one week prior to injury. Not sure how it got hurt. Then on 5/12/2009, Employee was pounding down a spout with a sledge hammer for about 45 minutes. Afterwards he felt alot of low back pain from pounding. Reported to nurse and iced went to doctor afterwards. Employee recieved 10 lbs lifting restrictions.

Struck by flying object

Employee reported injury while guzzling 2 days after it happened. on 4/8 The Plant Nurse looked at his eye it was red, irritated, and had a visible abrasion to it. She instructed employee to see eye doctor immediately. Employee was given antibiotic drops to take 4x's a day and was instructed stay out of work environment until 4/9/2009.

Employee was in the 900 loader cleaning out under the 11 grade bin. When employee turned his head to back up he felt a sharp pain in the back of his neck. Went to doctor, got prescription medication and on restricted duty.

Miner was sharpening a hook knife away from his body, knife slipped off the sharpener and cut his finger through the glove.

Caught in, under or between a moving and a stationary object

Employee went to close dump truck tailgate and crushed ring finger on right hand requiring 14 stitches.

Employee was using utility knife to cut strap. Knife slipped and employee cut his arm.

FTE was cleaning scrubbers. He climbed the 3' ladder; hit his head on the overhead pipes. He said he was ok. FTE was asked if he wanted to go to clinic, he wanted to wait until Monday. On Sept.14 EHS Manager did a follow up, FTE feeling better. 10/2 FTE wanted to go to clinic, paperwork received on 10/5, with prescription.

Fall to the walkway or working surface

He was dumping a wheel barrow in #1 elevator, after wheel barrow was empty, he stepped back & fell. He believes he stepped on the side of his foot causing the fall. He scraped his elbow, no other injuries reported. He washed his elbow off & he said it was fine, but just wanted to report it. On June 30th employee went to doctor becuase it was infected. Antibiotics administered.

Installing liner with winch and snatch block. Pulling liner into position. Left hand placed on winch cable. Load jumped further than expected and burr on cable caught glove pulling tip of left pinky finger in snatch block. Crush injury of tip of left pinky finger.

Employee was replacing an air actuator valve. The valve was held up with a piece of clothes hanger. The employee cut the wire, the valve swung free and struck the employee, chipping the upper right second and third teeth.

Rubbed or abraded

Maintenance mechanic was kneeling and reported pain and swelling in right knee. Employee reported the incident immediately. Medical treatment was received on 2/5/10 as employee did not want to see a physician prior to this date thinking ""it was just his arthritis.""

Exact cause of injury unknown. Employee alleges that he felt pain in right knee as a result of getting in and out of vehicles (i.e., bobcat) and because of walking, including mixing and bulk clay.

Employee noticed soreness/pain in right wrist when shoveling and sweeping. Employee stated pain is still in wrist with sharp pain going into forearm and his hand falls asleep. On 9/9/2009 employee went to doctor and recieved prescription medication and a splint.
