Large grass fire started from sparks that were generated from Savage rail pulling cars along 101 rail near area called Granma's Crossing. Fire quickly spread due to very dry conditions several fire dept. engines responded and took about 2.5 hrs to contain area. Small areas continued to smolder at 20:42 hrs and 3M along with Fire Dept. monitored area so there would be no flare ups.

Employee was building rotex screens and he told his supervisor that his hip was sore. He went home & called the supervisor a few hrs later as said his hip was still sore and that he would ice it down.
driver picked up wheel chock felt pain in left lower back. Was ok until sat 24th and he woke up with a lot of pain went to doctor and he was wrote of work for one day. then sedentary for until next visit. Prescription meds administered as well
Employee stated that while loading a bulk clay bag onto the bulk clay skid, he felt a strain in his shoulder. He placed the skid on top of the new bag, and was securing straps to skid. Employee felt he had enough clearance to reach in and secure the third strap. He used his right hand crossing over his body and reaching the strap to pull it, causing a strain in his shoulder.
Employee was walking down to the basement and he missed the last step and he lost his balance. When he lost his balance he fell, fracturing his lower leg.
In the cource of The employee's normal work and over time the employee has had pain on his Left middle finger he believes with discussions with the Nurse that his normal work activities have contributed to this condition or pain. His finger does not move to straighten out. Surgery on 3/2/2012
Employee was guzling rock next to a conveyor and when he pulled on hose his elbow hit metal railing. Bumped right elbow employee did not go to doc until 11/6/2012
Employee was opening rail cars and when he went to open a rail car, he started to open the car and felt a pull in his back. Normally the cars are empty when the employees go to open the doors. The employee asked another employee to look inside from the top to see if it was jammed and that is when it was found that the rail car still had approx. 30 Ton of rock inside.
Employee twisted knee on 5/18/2011, Employee had surgical repair of torn meniscus left knee.
EE REPORTED HIS RIGHT KNEE WAS HURTING AND GETTING WORSE. HE THINKS IT HAPPENED A WEEK OR 2 BEFORE (HE DOES NOT KNOW WHEN FOR SURE) WHILE HE WAS CLEANING THE SILICATE TANK. HE SAYS IT MIGHT OF HAPPENED WHEN HE WAS JACK HAMMERING. HE DOES NOT KNOW FOR SURE. HE JUST WANTED TO REPORT IT. Employee went to doc and has restrictions
Employee had his left hand little finger smashed between the frame of a crusher and a very large magnet(several hundred lbs). This created a laceration that required 9 stitches to close and a hair line fracture.
Mechanic was going to increase amperage on a welder, on his way to the welder he stumbled over a bolt left in the floor from old construction and fell face first into a concrete pillar. The incident caused a laceration to the right cheek requiring eight stitches.
Individual lost his balance and reached out grabbing the top of a chain link fence. The sharp barbs on the top edge of the fence lacerated the palm of the right hand.
Jackhammer task in silicate tank clean out over 8 hour shift. Following morning very stiff back muscles, strain. Employee went to doctor week Later and they prescribed an anti inflammatory.
While employee out on a non work related illness he was diagnosed with bi-lateral CTS.
