Struck against a moving object

Employee was plowing snow in a front end loader when the blade of loader came in contact with a rail road rail. The loader stoped suddenly and jarred employees low back.

Employees were installing new ducts for transfers points, one of the ducts got jammed ee kicked the duct to free it up, thus hitting him in the upper body. this was no damaged done to equipment.

Employee parked work pick up truck near primary in parking area. When they proceeded to exit the truck they stepped from truck and onto lifting eye on hole cover plate on the ground. As they tripped they overcompensated to catch their fall. In the process they tweaked their lower back, never falling to the ground never the less still sustaining back discomfort.

Miner bumped his head on a cross beam while closing a slide on baghouse cone. Strained neck. Miner was wearing all proper PPE.

Employee was conducting an inspection of truck outdoors and slip on some ice and strained right knee. he returned to work and just found out on 4/5/07 that he needs to schedule surgery for torn miniscus. Employee is still working full duty at this time

Felt pain in elbow while mixing. Felt better during rotation for 2 wks., began hurt again when rotated back to the mixing position.

He climbed up on vehicle using handle and foot slipped. Employee continued to hang on handle. Felt a (pop) strain in right bicep muscle.

Employee was trying to loosen a fitting on a water valve, and could not loosen it. Another employee was helping him and was using a second pipe wrench. The wrench slipped and pinched the middle finger on the left had of the employee, between the 2 wrenches.

Employee was taking his lock off #24 Rotex disconnect box & when he turned around to walk away someone had left the #3 roll bill door open & he fell through the opening approximately 15 feet into the bin. Employee sustained a laceration to the right knee requiring 15 staples.

Driver stepped out of truck and his foot slipped off of the truck step. Twisted his back.

Over-exertion in lifting objects

Employee was attempting to prevent a bag from falling to the floor at which time he felt a strain in his back.

Employee was using a fulcrum to raise a cage up so a co-worker could cut the wires of the cage. The empolyee had to maintain pressure on the fulcrum. As the employee was applying pressure, the co-worker cut the last wire. The handle hit the tube sheet and the employee's finger was pinned between the handle and the tube sheet.

Inhalation of radiations, caustics, toxic and noxious substances

Employee suffered respiratory distress and removed their respirator in a respirator required area. The employe was then exposed to cuprous oxide. The employee then experienced pneumonia like symptoms.

Employee was trying to prop door on bottom of #14 Elevator with broom stick handle, handle brke & hit employee on right hand causing small cuts & swelling.

Struck against stationary object

While changing a light bulb the light bulb broke, causing a laceration on the index finger of the left hand. The employee saw the nurse, she sent him to the Urgent Care and he received 3 stitches.

Over-exertion in lifting objects

employee was helping the driller use the hydraulic wrench to remove a drill steel. While lifting the wrench, felt a pull in his shoulder.

Employee refused medical treatment @ the time of the incident he didn't feel it was sever enough to warrant medical attention, 11/13/07 returned to nurse requesting medical attention He was tightening a bolt on an HP300 Head whe the wrench slipped hitting the employee's little finger on right hand on the inside of the head.

Over-exertion in lifting objects

Employee started at 12 noon on the day of the accident. He was moving a piece of a frame to the rotex. He lifted it and that is when he felt the pain in his wrist. He waited to report it until 9/12 at 6:00 PM. It was at that time he was sent to the clinic.

Fall to the walkway or working surface

Employee was carrying two baghouse cages one in each hand while standing in elevator. Elevator came to top floor and did not level completely. Employee tripped on floor and fell on left knee. Strained it as well

Struck against stationary object

Mechanic was removing transfer case from 992 weld truck. When transfer case was supported by jack and ready to be lowered, the transfer case shifted slightly. He pulled his right hand back towards his body and caught a sharp edge or burr on the underside of the front part of jack. The employee lacerated the right thumb palm of hand requiring 10 stitches.

Miner operating forktruck moving pallet with screen crates. Forks not completely under load and load not tipped back. Pallet fell, causing crate to break. Miner had puncture wound on (L) palm by nail protruding from broken crate. EE was not wearing gloves.

Slurry of pigments splashed in right eye. Using air supply to flush solidified mix at bottom of mixing pot. Operator was wearing PPE. Went to hospital doctor flushed eye and administered a perscription medication.

Employee was drilling a plate for a pump stand in the maintenance fab area and some how he got his right hand with a glove on it tangled in the rotating drill bit. Which twisted his hand and gave him a few small cuts and a dislocated ring finger. Took him to the medical center they gave him first aid reset finger.

Struck against a moving object

On loaded trip with finish truck, he crossed over train tracks to slow and jarred his back. This happened about 6pm on 6/14/07. He reported it on 6/20/07 when treating it himself didn't work. (ice & aspirin). At the time he didn't think it was anything to worry about, but pain did not go away.

Employee had just sent a sample and was coming across 11 conveyor on the crossover steps. On the last step coming down his foot slipped off the step and with his left arm he caught himself from falling by grabbing the rail with his left hand. His right spun him around which hurt his left shoulder.

Employee went to the nurse on June 6, 2007 and stated two weeks prior his right knee started to hurt, due to driving the 936 and 988 loaders. He states that the rough drive and having to push hard on the pedal is making his knee hurt. Nurse treated and will have to due follow up.

Struck by falling object

An employee was moving a rotex crate. As he we was moving it he lost his balance the crate fell. As the employee attempted to steady himself, he grabbed onto a cart that was behind him. The crate fell on his finger which was on the cart.

EE was ascending an extension ladder when supervisor went over to hold the base of the ladder, the ladder slipped because the hooks were not engaged, therefore pinching right thumb causing laceration of thumb.

An employee got dust in his eyes while engaged in work outdoors.

Over-exertion in lifting objects

Picking up dryer flight and felt a strain in low back. Was informed by nurse that he was being treated by a chiropracter. No lost or restricted time. Just went to chiropractor a few times and adjustments were needed.

Struck by falling object

Granule leaking from chute. Employee was closing the leak when granule fell behind his safety glasses and got into his eye. Wearing all PPE: Safety glasses with side shields, hard hat, Respirator.

While changing the screens on a Rotex Screener, the employee dropped the lid on her finger.

An employee was dragging a rotex hopper across a grated floor. As the employee was dragging the hopper, it snagged in the grating. This caused the employee to drop the hopper. The hopper then fell on the employees foot causing it to fracture.

Possible exposure to copper oxide causing rash on forearms & neck. EE not aware of specific time(s) of incident possibly causing the rash.

Miner claims being jarred around in primary loader caused c/o left wrist and left elbow pain/numbness and pain also in right wrist.

Employee was subjected to an occupational cumulative granite dust exposure over the years.
