He was gathering a sand sample. He inserted sample tube into the the sample pad of sand, and it stopped abruptly, striking him very hard on his right forearm.

He just bent over to pick up a bucket of sand in the lab, and hurt his back.
EE was working on Plant 4 screen and said he ""Bent his knee wrong"" felt like he twisted it Saturday April 24th. On April 27th an accident report was filed. EE went to see a physician from our Workmens Comp panelists who referred him to an Orthopedic specialist who recommends surgery scheduled for June 22nd 2004.
Employee was stacking bags of lime and twisted lower back, which caused a strain. He is currently being treated by a chiropractor daily.

mechanic started acetylene/oxygen blow torch, heard loud popping sound, turned it off, it ""blew up"" near hose. his left palm, side and back of hand was burnt. He performed first aid on himself ,we sent him to Tyrone Hospital where they prescribed him pain relievers, antibiotics and wrapped his hand. torch was rebuilt approx. 2 years ago by National Valley Gases.
Employee was cutting trees when the chainsaw he was using slipped and cut his left knee. He then went to Nason Occupational Health where he got two stitches.
He was replacing ceramic tiles, breaking them away with a chipping hammer, a tile splintered and a piece went into his middle finger on his left hand.
Employee was working on transmission on Euclid water truck when a piece of metal entered eye.
He was taking the crusher apart using a hammer and wedges. A piece of the wedge broke off and stuck in his cheek.

The employee was loosening a bolt on a cylinder on a payloader when he hit his elbow.

EE WAS WORKING NEXT TO WELDER. THE WELDER WAS USING A CARBON ARC TORCH, CUTTING WELDS APART & APPARENTLY THE EE WAS CLOSE ENOUGH TO THE ELECTRIC ARC TO RECEIVE FLASH BURNS.
