Struck against stationary object

Employee was in the process of greasing 988F loader. As he walked around the loader, he stepped on a rock that rolled under his foot. He sustained a sprained right knee and it would not allow him to walk.

Struck against a moving object

Left side groin area started hurting while flipping a headnut for a crusher.

Fall from machine

employee was using ladder on side of tri-axle truck to check inside the bed when he lost his footing and fell to the ground. he landed on his feet and rolled back-wards, injuring his back.

Caught in, under or between a moving and a stationary object

Miner was using an impact wrench backing a bolt out of a screen deck when the length of the bolt caused the left hand of the miner to be pinched against the top area of the screen deck where he was working.

While running the bulldozer, the employee developed chest pains which ultimately were diagnosed as a heart attack.
