Emp was using a digging bar to help loosen a tire on a haul truck. Bar slipped and struck employee in the shoulder area.

While employee was tearing down Hatachi 1200 Hoe , He was using a wrench and his right arm locked into a bent position. He used his left arm and straighten out the right arm. Finished out the day working. Worked out the balance of the week and went to Occupational Health that Friday 11/02/2012

Insured worker and another emp were unloading 40ft long pvc pipe when emp lost handle on pipe and it hit him on his right knee. Emp missed 3 days of work. Emp returned to work on 10/13/12.

Struck against a moving object

The Hoe Operator was loading a large rock onto a Euclid when the rock slipped from the hoe bucket and fell onto the bed of the Euclid, jolting the Euclid driver causing strain to his neck, shoulders and lower back.

Over-exertion in lifting objects

Employee was bolting teeth onto a loader bucket. When he picked up the teeth, he felt a pain his groin area.

Caught in, under or between a moving and a stationary object

The EE was exiting the track hoe when the door blew shut causing a minor wound the his R leg just below the knee area. The injury exacerbated an existing condition causing infection. The employee didn't tell the site supervisor until 3/15 and it was not reported until 3/16.

Employee struck his right hand while hitting a bearing with a hammer fracturing his finger.

While EE was spraying out tunnel aggravated a shoulder injury. Left should sprain.

emp was making repairs on plant and was holding onto an i beam, emp felt his right wrist pop. emp reported the claim on 9/26 as record only but on 10/1 made an appt to see dr on 10/02. emp has a broken wrist

Employee hurt his lower back while lifting and twisting as he tried to change a tire.

Employee was lifting screens in the quarry and strained his lower back causing a herniated disc.

Fall to the walkway or working surface

A general laborer fell into the surge pile while cleaning materials from the feeder opening and became trapped.

Emp was installing wear rubber on vsi when lid was lowered, pinching his right forearm.

Struck by falling object

EE was helping install cab protector on a truck when it slipped off chain and struck emp in the right shoulder.

Over-exertion in lifting objects

As the EE was helping to repair a crusher he lifted one end of a pallet that contained machine parts as he was performing this task he sprained his L shoulder. The incident occurred on 9/7/12 but was NOT reported until 9/10/2012.

As the EE was tighting a plow bolt on a quarry loader his L hand came into contact with the bolt head and the impact wrench caused it to spin and lacerate his L palm.

Repairing conveyor belt using homemade belt clamp, when removing clamp, the clamp closed and pinched right ring finger between two pieces of angle iron, lacerating nail area of finger.

While performing maintenance EE knelt down on catwalk and knelt on a stone that was lodged in the expanded metal, causing a bruise on the L knee.

Employee was helping repair plant when left ring finger got pinched between pry bar and metal frame.

aggravated tendonitis in right shoulder unloading a box from delivery truck

Cleaning out silo with shovel, scraping and shoveling, strained lower right back.

EMPLOYEE WAS OPERATING A HAND GRINDER WHEN THE CUTTING WHEEL SHATTERED. THE GRINDER WAS KICKED OUT OF HIS HAND, CUTTING THE 4TH FINGER OF HIS LEFT HAND. THREE SUTURES WERE REQUIRED TO CLOSE LACERATION.

Employee was trimming a conveyor belt and cut his right hand with the utility knife.

Struck by flying object

Miner was grinding steel and F.B. entered Lt eye under safety glasses-required follow up treatment after continued irritation.

Struck against stationary object

Employee had his lower right leg lacerated when his leg made contact with a screen cloth during maintenance on a screen deck. Sutures were required to close the laceration.

Struck against stationary object

As EE was tightening the bolts on the pay loader bucket, the 1"" air wrench slipped off one of the bolts striking his right hand between the air wrench and pay loader bucket. Bruising the tendon of the second finger in the right hand .
