Caught in, under or between a moving and a stationary object

While cleaning behind the bearing journal the come-along slipped allowing the load to drop onto the employee's right hand, injuring their thumb and index finger.

Over-exertion in lifting objects

EE was changing seat in a cat 777D rock truck. EE removed bolts from the seat, and lifted it to the ground using a crane. EE then took seat into shop with dolly cart. EE went to flip the seat over in the shop to remove the suspension from the seat body and felt discomfort in lower back.

Struck against a moving object

Two employees were driving the maintenance truck down the ramp to the bottom level of the primary when the brakes went out. The truck was driven into a berm to come to a sudden stop. Both employees showed no injuries at the time of accident. One employee sought medical attention for lower back on 11/5/2019.

Struck by falling object

An employee was cleaning material out from under the primary conveyor so that EE could wash down the area that was unable to reach with a shovel. As EE bent to pick up a large rock, a second rock came off the conveyor from above EE and hit EE's hand. Xrays showed it's fractured.

Two employees were mounting a storage cabinet to a wall inside the tool trailer. One employee turned to pick up an impact drive on the work bench and felt discomfort in lower back and hip.

Unloaded top piece of crusher, sat it on jersey barrier, top piece of crusher shifting catching employee's foot between rim and ground

Employee sustained a crush injury to the left middle finger while removing hardware that secures the top of the crusher to the base of the crusher.

After cleaning mirror on drill cab, lost right footing on step, left foot got stuck in cab step and lost grip on hand bar. EE fell and hung by the left foot. Witness heard EE and removed EE off drill.

Small stone on landing surface caused right ankle to twist, was working at top of primary crusher

Employee was dislodging frozen material from #1 Bag House when EE bumped right wrist causing a sprain.

Employee was using a box cutter to cut rubber to put around a hydraulic hose when the box cutter slipped and struck the employee above the left knee causing a laceration requiring stitches.

Employee was emptying slam pan in shake room when pan slipped and cut thumb. Employee was not wearing gloves.

Fall onto or against objects

The employee was climbing down the ladder on a loader when they lost balance and stumbled backward. As they were falling to the ground, they pulled a chain and posts down with them. One of the posts struck them on the forehead causing a laceration requiring stitches.

Struck by falling object

Employee was welding underneath truck with welding helmet and safety glasses on when buildup dirt fell from the bottom of the frame and dropped down onto the shield and scratched their left cornea.

Fall to the walkway or working surface

EE placed chock block under wheel of pickup. EE's chock block is attached to the truck with a rope. EE tripped over the rope and landed on right side injuring right upper arm.

Fall onto or against objects

From a squatting position on the walking platform the mechanic began to remove the middle housing that contained the high pressure filter. After breaking the housing loose, the mechanic turned the housing by hand. The housing fell loose into EE's hands. The weight off the housing, filter and oil cause the employee to fall forward into the scalar from chipping a tooth

Struck by falling object

The employee was changing worn counterweights in a cone crusher. The crusher base had some sand still stuck to the side walls of the crusher. When changing weights, dust fell into the employees eye causing a scratched eye.

Contact with hot objects or substances

Employee was using a torch to remove rusted bolts on the middle screen deck when hot slag entered left ear causing a perforated ear drum. Employee did NOT seek medical treatment until April 10, 2019.

Contact with heat

High humidity and temps in the 90s caused the individual to become dizzy, short of breath, light headed, and have an irregular heart beat.
