After coming to a stop & looking both ways, EE crossed the road in the water truck. A passenger vehicle traveling at a high rate of speed hit the water truck at the front right tire area. The EE was taken to a health works facility where EE was diagnosed with a contusion and swelling on EE's left wrist.

EE was climbing up a ladder at the classifier when EE heard the right knee pop. The EE was not doing any unnecessary twisting on the ladder while maintaining 3 points of contact. EE indicated did not have leg positioned straight, with EE's foot at a slight angle to the rung when EE applied weight to it.
While performing normal job function of operating a front end loader with joystick controls, the EE felt a slight pain on the top of left hand. About a minute or two later, the top of hand started to swell. The EE was taken to the ER where doctors determined they had a hematoma which was caused by a small broken blood vessel.
EE was walking down a slight incline along the side of a roadway, EE slipped on loose stones and fell forwards onto EE's knees as EE fell, EE caught self with EE's right arm spraining EE's right shoulder and causing a contusion on both knees
EE was using a bar to dislodge blockage from an oversized chute. When the blockage released, rocks hit the bottom of the bar, pulling the top end into the chute with the EEs right hand causing a fractured pinkie.
EE was shoveling dirt from the floor onto a non-moving conveyor in the surge tunnel and suffered a muscle strain in upper thigh
EE was wiping dirt from rear hazard strobe on top of tank during pre trip inspection. While climbing down the ladder, EE's wedding ring became caught on ladder rung causing a deep laceration to ring finger on right hand.
EE was climbing down the 4 rung folding ladder for conveyor belt 102 when the right foot became pinched between the last rung and a washer used to secure the ladder in an upright position when not in use. The EE fell to the ground landing on right elbow which put strain on right shoulder causing internal tissue damage to the joint.
Employee was standing on a 988 front end loader between the bucket and loader arm standing on the links. Employee stepped back to observe employee's work and fell off the machine, injuring midback/ribs.
The employee was attempting to put an oxygen bottle away for storage, employee put the bottle on end and turned away for a moment, the bottle topped over striking employee's foot.
The injured employee did not see the small pile of stone which caught their foot when turning to get tools from the service truck.
Employee was cleaning a box under a conveyor that was shoulder high. EE was using a 6-foot digging bar when EE felt a pain in EE's right shoulder requiring medical examination and lost time. Employee returned to work 9/23/2020
Winch got bird nest from cable being frozen on boom overnight and was in process of rewinding cable back on drum when crowbar slipped and EE fell back 4.5 feet of counterweights onto ground on outrigger pad mat.
Operator was working on a platform, removing a crusher, when EE stepped in an opening in the floor, EE lost balance and caught self on a railing with rib cage and knee. EE did not report the accident until 9/8/2020
EE was removing rock from crusher jam due to a deflector plate falling over the opening of the cone crusher at the end of EE's shift. EE reported pain on Monday 8/24.
EE stepped on loose rock, lost balance and cut right knee when EE fell.
Employees were installing a tail pulley on conveyor 13. The tail pulley became wedged between the framework of the conveyor. When an EE pushed on the conveyor to dislodge it, it was loosened and pinched the EEs finger between the pulley and framework. This resulted in a fracture to the EEs right middle finger.
EE was repairing a rear strobe/tail light on a Tri-Axle truck. EE removed cut resistant glove from Left hand to separate light fixture from wiring harness. EE then attached new light fixture to the wiring harness and pushed light fixture into position when EE's left 5th finger struck the tailgate slider causing a laceration.
EE WAS CUTTING V-BELTS THAT HAD BECOME ENTANGLED AROUND THE MOTOR SHAFT OF THE HP - 300. THE BLADE OF THE UTILITY KNIFE EE WAS USING SLIPPED AND CUT THROUGH THE EMPLOYEE'S GLOVE CUTTING EMPLOYEE'S HAND.
EE was standing on the ground welding at face level wearing safety glasses, a face shield, and mask. EE stopped grinding lifted EE's face shield to speak to another EE. EE felt dust enter EE's eyes. EE immediately used water to flush the eye, Next day EE's eye felt fine. A few days later it became irritated. The EE did receive medical treatment to have a piece of old steel removed.
EE developed tennis elbow while removing bolts from a jaw crusher. EE informed management of the injury on 11/11/2020. EE received treatment on 11/13/2020
Miner experienced a reportable threshold shift in hearing levels when compared to EE's baseline audiogram which met MSHA reporting criteria.
