Using razor blade knife to cut pump cable.

EE was installing a thrown track on a D10R when a job mechanic struck metal on metal with a sledge hammer resulting in a metal shard puncturing EE's chest.

EE left work on 02/02/2016 without reporting an accident. EE reported to the hospital the following afternoon and was treated and released for a back sprain. Dr excuse said to return to work the following day 02/04/2016. EE has not yet returned to work. EE did call on 02/15/2016 and said EE would return next day. Has not returned to work as of 02/21/2016.

Employee was closing open doors on train cars, prying them shut with a bar, when bar slipped out of door, releasing pressure on bar causing employee to stumble, causing pain to shoulder, back, & neck area - On 1-6-14 EE was treated in the ER with a return to work with no limitations. On 2-7-14 EE was seen by a specialist. On 2-10-14 EE would be off work for 3 weeks.

Repair man changing boom swing jack on continuous miner. Trying to loosen hose fitting when wrench slipped and cut left hand. Went to doctor had four stitches. No time lost due to injury.

Employee was breaking a track on a D-10 T Dozer, striking the Cat with a hammer with one hand & had his other hand resting on track, Track moved, catching his finger and mashing it. Received 4 stitches and tore fingernail off.

Driving bearing race into steering stabilizer hammer chipped puncturing left hand.

Employee was using a pry bar, the bar slipped and struck him in the forehead.

Employee was shoveling underneath head drive when his finger came in contact with a piece of metal, resulting in 3 stitches being put on left right finger.

The employee was using a pipe wrench to move a sheave to put the belts on a drive when it slipped and caught the tip of the right middle finger between the pipe wrench and the sheave.

The employee stated that he was removing a breaker box cover and he had his knife in his right hand and it was open, the cover started to slip so he let go of the knife and it fell striking him in the left thumb cutting the bottom side of his thumb.

The employee was using a crescent wrench to tighten some blots when the wrench slipped and his right hand came in contact with a piece of expanded metal. The sharp edge of the metal cut his right hand around his knuckle area.

Employee was extracting a bearing out of the hardbar on a 475 Komatsu Dozer using a hammer. The bearing released, twisted, and caught the employees right hand causing a laceration requiring six stitches.

The accident victim was shoveling loose coal along the belt line. When attempting to unload the shovel, it came in contact with the belt line causing the shovel to wedge against the mine roof, straining the victims shoulder.

Preparing a hyd hose for fitting when the knife slipped resulting in a cut to the left thumb.

Wrench slipped while tightening poly-belt that runs between motor and gear reducer on incline belt of highwall miner. Resulting in a laceration over his left eye that required stitches to close.

Employee was inserting the fitting ends in hexagon drill steel and struck his left index finger with a hammer.

While removing a track link from a D8R, employee hit the old link with a hammer. A sliver of metal came off the link and penetrated his right arm.

Employee was lifting a belt roller with a slate bar while another employee was trying to adjust the height of the roller. The slate bar broke striking the employee in the groin area.

Employee stated that he was loading roof straps into the bucket of his scoop. He was using a hammer to break the metal binding straps that were holding the roof straps in a bundle. The employee stated that on one swing the hammer missed the binding strap and as a result his right ring finger hit the binding strap causing a laceration requiring 3 stitches.

Employee was using a wrench, the wrench slipped off the bolt, he cut his left hand when it struck a piece of metal.

EE was attempting to knock a broke head jack pin out of miner by using a bar and a 16lb hammer. The hammer kicked the side of the bar and his hand struck the lid on the miner, resulting in a cut to his right pinky finger.

EE was holding a hammer against a pump while another EE was hitting with another hammer to drive a pump back onto a bolt machine. A piece of metal broke off, striking EE in the hand.

EE was assisting the core drill operator and as the drill started he was loosening a bolt up, it spun around and mashed the little finger of his left hand.

Injured was using hammer to strike a metal pin - part of hammer head broke away and struck employee in face.

EE was cutting a piece of beltline at a splice. He was holding the belt with his right hand and cutting with his left hand. He cut through the belt and cut his right index finger.

EE was standing beside feeder, watching a co-worker use a come-along to put conveyor chain back together, when the chain slipped & hit EE on the right cheek, causing him to get 7 stitches.

EE was using a slate bar to tighten up bolts when it slipped and hit him on lip and chin. He had (11) stitches put in his lip and chin.

EE was cutting zip ties loose that were holding cable in place on the high wall miner. When he cut a zip tie the knife continued on and cut his left hand. EE rec. 4 stitches.

Employee was using a sledge hammer to remove a tooth off a loader bucket of a 994. A piece of metal flew off and penatrated the left knee. This caused a lot of bleeding and surgery to remove the imgesef piece of metal.

While assisting the mechanic make a splice on the miner cable, EE cut his left wrist with a belt knife, requiring 4 stitches.

EE was putting chains on a motor grader using a hammer. While striking the chain roll pin, a piece of the hammer broke off and struck the EE in the bottom of the right eyelid. A sliver of metal was removed by a doctor; glue was used to seal hole in right eye-ball.

Employee was working on the belt line, using a metal bar to change out a roller when the bar slipped hitting him in the mouth.

EE was assisting the mechanic put a motor on the highwall miner using a porta-power when a metal brace broke loose and struck him on the right shoulder. EE continued to work for several hours before seeking medical attention.

EE was cutting a piece of water hose with a pocket knife and cut the top of his left thumb nail. Did not miss work, cut required stitches.

EE was helping others replace springs pads on 800 Mack truck (ST140), when one of the jacks kicked out and caught ee right index finger between spring and crib block.

Knocking bits out miner lugs using hammer. Hit finger on bit lug & hammer handle.

TRUCK DRIVER WAS TIGHTENING HIS WHEELS WHEN HE TURNED AND TWISTED HIS ANKLE.

While shaking roll-feed gate the chain support broke a link, causing the bar he was using to slip. Injured fell on right foot.

Was knocking troughing roller back with 16 lb hammer when roller stuck injured on left leg.

As employee was finishing a belt splice, the come-along being used to pull the belt together slipped and struck him in the left shoulder.

While cleaning under the belt tailpiece, employee was using a slate bar to dislodge compacted coal and hit his thumb between the slate bar and the tailpiece.

EE was splicing a tape line and cut his hand.

Injured was knocking gate in on refuse chute, missed gate & hit his thumb between hammer handle & handrail rod.

Working on hose red chain. Trying to take apart, wrench slipped, causing hand to be cut on palm of hand (L) side.

EE installing drive chanin link for the conveyor chain, using hammer and a chisel. Struck chisel w/hammer and a piece of hammer or chisel went into left forearm. Caused spurting of blood from left forearm. EE went to Welch Community Hospital, assisted by another EE.

While shoveling the belt, EE was pushing against the shovel with his right knee and experienced pain. After consulting a Dr., it was determined he had a sprain/bruise.

Employee was shoveling belt line when he allegedly hit his hand against bottom structure. *Dr. stated this condition could be from an old injury.

While installing pins in loader teeth a piece of metal from the pin struck employee in groin area.
