Showing 50 of 53EE was installing a thrown track on a D10R when a job mechanic struck metal on metal with a sledge hammer resulting in a metal shard puncturing EE's chest.
EE left work on 02/02/2016 without reporting an accident. EE reported to the hospital the following afternoon and was treated and released for a back sprain. Dr excuse said to return to work the following day 02/04/2016. EE has not yet returned to work. EE did call on 02/15/2016 and said EE would return next day. Has not returned to work as of 02/21/2016.
Employee was closing open doors on train cars, prying them shut with a bar, when bar slipped out of door, releasing pressure on bar causing employee to stumble, causing pain to shoulder, back, & neck area - On 1-6-14 EE was treated in the ER with a return to work with no limitations. On 2-7-14 EE was seen by a specialist. On 2-10-14 EE would be off work for 3 weeks.
Repair man changing boom swing jack on continuous miner. Trying to loosen hose fitting when wrench slipped and cut left hand. Went to doctor had four stitches. No time lost due to injury.
Employee was breaking a track on a D-10 T Dozer, striking the Cat with a hammer with one hand & had his other hand resting on track, Track moved, catching his finger and mashing it. Received 4 stitches and tore fingernail off.

The employee was using a crescent wrench to tighten some blots when the wrench slipped and his right hand came in contact with a piece of expanded metal. The sharp edge of the metal cut his right hand around his knuckle area.
Employee was extracting a bearing out of the hardbar on a 475 Komatsu Dozer using a hammer. The bearing released, twisted, and caught the employees right hand causing a laceration requiring six stitches.
The accident victim was shoveling loose coal along the belt line. When attempting to unload the shovel, it came in contact with the belt line causing the shovel to wedge against the mine roof, straining the victims shoulder.

The employee stated that on one swing the hammer missed the binding strap and as a result his right ring finger hit the binding strap causing a laceration requiring 3 stitches.
Employee was using a wrench, the wrench slipped off the bolt, he cut his left hand when it struck a piece of metal.
EE was attempting to knock a broke head jack pin out of miner by using a bar and a 16lb hammer. The hammer kicked the side of the bar and his hand struck the lid on the miner, resulting in a cut to his right pinky finger.
EE was holding a hammer against a pump while another EE was hitting with another hammer to drive a pump back onto a bolt machine. A piece of metal broke off, striking EE in the hand.
EE was assisting the core drill operator and as the drill started he was loosening a bolt up, it spun around and mashed the little finger of his left hand.

EE was standing beside feeder, watching a co-worker use a come-along to put conveyor chain back together, when the chain slipped & hit EE on the right cheek, causing him to get 7 stitches.
EE was using a slate bar to tighten up bolts when it slipped and hit him on lip and chin. He had (11) stitches put in his lip and chin.
EE was cutting zip ties loose that were holding cable in place on the high wall miner. When he cut a zip tie the knife continued on and cut his left hand. EE rec. 4 stitches.

Employee was using a sledge hammer to remove a tooth off a loader bucket of a 994. A piece of metal flew off and penatrated the left knee. This caused a lot of bleeding and surgery to remove the imgesef piece of metal.
While assisting the mechanic make a splice on the miner cable, EE cut his left wrist with a belt knife, requiring 4 stitches.
EE was putting chains on a motor grader using a hammer. While striking the chain roll pin, a piece of the hammer broke off and struck the EE in the bottom of the right eyelid. A sliver of metal was removed by a doctor; glue was used to seal hole in right eye-ball.
