After arriving on section employee checked face to see if any bolting was needed. After checking face then was walking back to belt set-up when fall occurred.

Employee stated that after evening shift was ended he slipped on stairs from mine to parking lot fell in ditch and injured knee.
Facts are at time of this report (taken from statement by Anderson Count Sheriffs Dept). 2 males on haulage road from mine got into a fight? and one shot the other with a 40 cal Taurus handgun. Victim is alive at this time. Neither man is employed by company.
While climbing off loader, employee slipped at top and fell, grabbing handrail with right hand midway down, stopping decent. This caused a sharp pain in right shoulder. Employee finished shift and continued to work until shoulder worsened on 1/30/12 and is now off awaiting release from doctor.
Reaching into parts bin to get tool and was bitten by brown jumping spider.

Scoop changing cables were stuck in mud and employee was pulling and jerking on them when he experienced a pain in lower abdomen.
an unplanned roof fall was found in the 1st SE mains at crosscut #14 of the No. 5 entry. The fall was in the intersection and approximately 30' inby in the entry. The fall was approximately 4'thick X 18'wide X 50'long. There was no injuries or property damage. The fall was discovered by an examiner. There was a worked out area.
While running Jay Miner softball sized rock was thrown by chain and struck person on right shoulder.

EE was shoveling coal off of belt #2 when a large rock fell on him crushing his foot.
Cut his right forearm. 1'x12""x20"" sharp edged rock hit him in the arm.
Intersection fell 20' x 20' x 5' high. North west mains #3 entry 3rd crosscut inby spad #37 old panel. Doesn't interfere with air or anything at mine.
EE was operating the miner setting on the rib when a piece of rock fell from the top and hit him on the right side of his head and shoulder. It fell about 3' where he was sitting. It was about 2'X 2' X 2"" thick.
Intersection, 1 break outby spad 172 fell. 20' x 20' x 5+'.
Employee was eating dinner when he got out of bolter deck, stood up behind bolter and a piece of draw rock fell on his shoulder & neck. Did not knock him down. Rock was app. 24"" long x 22"" wide x 2"" thick in size.

At spad EE on 1st right panel off 2nd NW Main had roof fall 5'high, 18'wide, 30' long . Fall did not interfere with air movement or anything else. On worked out section.
EE runs off side bolter, another EE pulled bolt up and set ATRS. EE moved to swing boom out to start drill, when a piece of rock approx 25"" X 36"" X 4"" fell on his lower left side and back.
Miner was cutting sandstone roll. A piece of cutting bit came off miner & hit employee in upper left arm.
Bolt men were suppling bolter. Injured EE was on offside reaching across bolter to place bolt supply. The operator thought he was out of the way and trammed bolter forward onto injured's left foot.
Knocking bits out miner lugs using hammer. Hit finger on bit lug & hammer handle.
Miner was not working properly. Had been trouble shooting rub rail at main breaker panel off. Was checking rippers, would not start. Decided to take panel off to check contactors, when pulled pin on front rub rail, rail fell off miner and hit left foot. Also turned his knee.
Fall in 1st rt. #4 entry L.O.B in intersection 20 ft x 20 ft 5 ft-6ft high is not block airway travelway or escapeway
Man was getting off mantrip, stepped in hole and twisted right knee.(right)
A roof fall in the #5 entry at the face of the first panel of #4 belt. It was 5 to 7 feet thick 20' wide and about 35 to 45' long. It was a mine fault.
A roof fall was found inthe return #20 x cut on # 2 belt h 5 entry. it had coal striks in with sand strikiing and soft topic. It was 46"" to 50"" high 20' wide 25' long and it was in intersection.

A roof fall was found x21 #5 entry & xcut 18 to 19. The one in 21 xcut was halfway in the break 4 to 5 feet high & 20 feet wide & 20 feet long. The one at 18 to 19 xcut was 20' wide & 6' high & 50' wide. We will go around it at 17 to 20 to #4 entry & put new brattice in it. There will be 6 brattice.
A roof fall was found inbetween #3 & #2 xcut on #4 belt at #4 xcut. It was 6' high, 18' wide & 50' long. Mine prop was put at end of fall & cribs at other end.
Roof bolter had a piece of draw rock that fell between ATRS and supported roof and it hit him.
Roof failed at xcut #23 in return on #3 beltline. The fall was a continuation of a previous fall. Unintentional roof fall.
Unintentional roof fall was found at 3:30pm on 4/21/08 at X-Cut #11 on #2 belt #1 entry (intake). Fall occurred in intersection. The roof fall was 4' high, 20' wide and 20' long.
Unintentional roof fall found by on-shift examiner and federal mine inspector at x-cut #14 in #4 entry on #2 belt line. Fall was 6' high, 20' wide and 60' long. Found at 1:30 pm on 3/11/08.

A roof fall in the #2 entry at #101/2 x-cut a wnt to #12 x cut on the #12 belt. It was about 7' to 8"" high 20' wide and 140' long.
A roof fall was found to have occurred unintentionally at X-cut 2 to 3 in #5 entry on #3 belt.
A roof fall was foung at #22 X cut in the #4 entry of #2 belt. It was approx 6'-7' high, approx 20'wide and 20' long.
Roof fall at #3 xcut on #4 belt in the #4 entry belt line. It's about 40' x 35' x 7' high. It looks like a large horse back.
A roof fall was found on preshift on # 4 belt it was about 4'or 5'thick, 40'l x 20' w. It had been heaving in the #13 area. It will be set with timber or crib, then start cleaning it up bolting it back.
Hill seam it was just bad top. Will set more cribs so it won't come on any father. it is about 20' w x 40' long x 7' h. It is at @2 xcut on #5 belt in between 2 block between #1 & #2. Found it coming outside at 3:30pm.
Roof Fall is in #11 x cut on #4 belt it is about 6' to 7' high 20' wide and 20' long.
#5 Entry fell in at #5 x-cut in Return. It is about 5 1/2' to 6' high, 20' long & in Entry section 300' inside of mine. Water is coming through the top & it had a hill seam in several places.
A 12'x 12' rock rolled down the mountain and came in contact with the fan and knocked the fan to the side and out of the entry.
He was coming out of mine at end of shift, getting off belt line, tripped, fell onto rollers and hit his knees.
A roof fall was found on preshift at about 7:00 am. It was about 7' thick, 70' long & 20' wide.

Cleaning off the belt line, raised up hit the roof of mine, causing a loose rock to dislodge and fall. It hit him on the left shoulder, knocking him down and falling to floor of mine.
Walking into mines and tripped on water line (hose), causing him to fall.
A roof fall was found at A130 when the #2 belt #2 section belt would not start to put coal on it.
Employee was getting out of the 21 SC shuttle car and twisted his knee:resulting in a sprained knee.
