A set up crew employee was attempting to remove liners from a cone crusher, by striking the liners with a 10 lb sledgehammer. During this process, a shard of the manganese liner chipped off and went into the employee's left forearm. The employee was taken to the emergency room where the shard was extracted, wound was cleaned, and 5 stitches were administered.

Worker was removing a high-pressure filter cap from a machine using a breaker bar. It slipped and crushed two fingers on worker's left hand between the breaker bar and hydraulic tank on the machine.

An underground blaster was cutting an ANFO hose with a utility knife when the knife slipped, causing a laceration on the employee's left index finger.

Employee was trying to loosen a 5-1/2' cone crusher after it had blown a hydraulic line going to the clamping system. The employee was using three, 3-ton come-a-longs to turn the bowl and loosen the crusher. As the employee was tightening one of the come-a-longs, it slipped from the rigging and came back and hit the employee in the right leg in the shin breaking the leg.

Employee was a using a 8 pound sledgehammer to hammer on the outside of a bin to knock down material that had built up inside. In the process of swinging the hammer EE felt a pop in the right shoulder. Employee was taken to a clinic and observed and released to full duty, no medications, and no restrictions. MRI reviewed on 3/23/22 showed no damage, but PT was recommended.

Employee spliced C1 conveyor belt and was tracking the belt when the laces came around and hit the strap that was holding the rear skirt rubber. Employee took a pry bar and pried up on the strip without locking out the conveyor first. Pry bar became wedged causing EE's left index finger to be smashed between the belt and the bar resulting in two lacerations on EE's finger.

Employee was in the process of installing the lid on a VSI. Miners were using a telehandle to lower the VSI lid back into place. The lid was slightly misaligned. Miner used a pry bar to move lid into place. As he began to pry, the lid shifted and dropped into place and pinched the bar causing it to flip up and strike the left side of the miners face.

Bearing housing bolts broke in scalping screen. EE replaced broken bolts. Nylon strap was attached to shiv which was standing vertical. Employee let go of the shiv to connect the chain come along to the strap. The shiv fell over & down a step hitting the top of employee's right foot. JSA had been done but falling hazard not identified. Medical attention not requested until 3-12-13.

Employee was tightening bolts on a pin cover plate. The employee felt a pain in his shoulder while tightening the bolts. He did not wish to see a dr at the time. He requested to see a doctor on 1/21/10 and taken on that date. He was told he needed an MRI and to see an orthopedic surgeon. The surgeon informed him on 2/24/10 that he had a torn tendon in his shoulder.

EE was attempting to remove the race from the shaft of a bearing when a piece of metal broke off and penetrated into his left forearm. Two stitches was the result of having to extract the piece of metal.

The ground man was attempting to dislodge rock with a 5' pry bar. When the rock disloged, employee was unable to stop momentum of the bar and struck his left hand pinky finger. Employee was taken to emergency room for x-ray, which revealed a small-angulated fracture on the little finger of his left hand. Employee returned to work without restrictions.

Replacing blowbars in primary impact crusher. EE's were using a sledge hammer in the process, loosening blow bars prior to removal. A piece of the blow bar came loose at it was hit with hammer. The fragment cut through blue jeans into EE's upper right thigh cutting muscle & nicking the artery.

Working on motor connections, cutting tape away from splice, sitting down in front of motor for access to equipment, employee was using "box cutter" type knife for tape removal. Knife hit wire insulation causing knife to jump up. Employee received a 2 inch laceration to left forearm requiring 5 stitches. Employee was released to return to work with no restrictions that same day.

Employee was cutting a belt to splice another section of belt in. Employees hand slipped downward on the utility knife that he was using to cut belt. His pinky finger came in contact with the blade causing a laceration to the finger.

Employee broke right hand while removing rocks, plugging impact crusher. Employee used 1/4 diameter chain to pull rocks from impact. As rock was out of the impactor employee positioned right hand around chain to guide to final placement. Chain slipped falling about 6 inches causing injury to right hand. Injury is a restricted duty.

EE ALLEGES THAT HE WAS PRYING A ROCK OUT THAT WAS WEDGED BETWEEN THE TAIL PULLEY AND CONVEYOR FRAME. THE ROCK POPPED OUT SUDDENLY AND THE EE STATES THAT HE OVEREXTENDED HIS LEFT ELBOW WHEN THE BAR WENT DOWN QUICKLY. EE ALLEGES HE FELT A PAIN IN HIS LEFT ELBOW.

EE was punching holes in pieces of belt with hammer and hole punch and had already punched out 6 holes. On the 7th hole he missed the hole punch with the 2 lb. hammer and struck his thumb on his left hand. X ray showed a hairline fracture on the knuckle. He was given a splint to wear. He was wearing gloves. Area was muddy and punch was too small to hold.

Two employees were repairing a tear on a conveyor belt. The tear had been cut out and the patch was being cut by the employee using a utility knife. As he was cutting the patch the knife slipped and cut his right calf muscle. The laceration required 5 stitches.

Employee was cutting slots in 1/2" x 8" skirt rubber. He picked the rubber up and was cutting towards his leg when he cut thru the rubber. He stabbed his leg with the box knife. The cut was about 1" long and 3/4" deep and took 3 stitches to close.

A BLAST HOLE DRILLER EMPLOYED BY INDEPENDENT CONTRACTOR GENERAL DRILLING WAS MAKING A REPAIR TO THE MUD FLAP ON HIS DRILL TRUCK. USING A PUNCH TO MAKE A HOLE IN THE MUD FLAP, HE MISSED AND HIT HIS LEFT HAND WITH HIS HAMMER. THE ACCIDENT OCCURRED IN THE QUARRY SHOP. HE WAS TREATED AND RELEASED AT A CLINIC. THE DOCTOR REPORTED "NO PERMANENT IMPAIRMENT". HE WAS RELEASED TO RETURN 9/29
