Employee was working in Texas on temporary project. EE was pulling a stuck wedge out of a broken block. When it came loose, the upward motion pulled towards another wedge and left thumb got pinched in between the two wedges, fracturing left thumb. EE is working light duty. No lost time.

Employee was prying a part granite vertical slab with pry bar, attempting to place a wooden wedge between the slabs, when the pry bar slipped, rapidly closing slab back up, pinching tip of left index finger, causing loss of nail. No lost time.

Employee was using utility knife, putting notches in the end of a plastic pipe to insert a string into a saw cut hole for wire sawing. Utility knife slipped and cut his left index finger from last knuckle to tip of finger. Received six stitches. Released to work, no restrictions. No lost time.

Employee was hitting excess granite off saw block using 10 pound sledge hammer. When he hit with hammer, granite bounced off, cutting right side of his face, just above the lip. Sutures placed. No lost time.

Employee was chiseling hard packed dirt off of the stone he was drilling, using a 5' crowbar. He was swinging/twisting to his right with the crowbar and his back gave out. He went to the chiropractor and returned to work later that morning. No lost time.

Employee was wedging stone, using a 60# wedge driver on 5' (22#) wedges, then pulling wedges out again. Does this repeatedly all day long. Felt pain in lower back, left side.

Employee was barring slabs and rough backs apart using a pry bar. He felt something pop in his lower neck, upper back area. He didn't seek treatment right away but as he continued to have pain in upper back-lower neck area, he saw a chiropractor on 4-19-10, was adjusted and released, returned to work. No lost time.

Employee was on quarry loader catwalk prying with a bar to get bolt out. Bar slipped out of bolt hole, caught his head between tire and catwalk. Laceration right ear tip, sutures applied.

Employee was separating slabs, steel bar slipped and stone fell back onto bar which in turn jerked and pulled employee against direction he was pulling. This caused strain to right arm/shoulder and to right mid back directly below shoulder blade.

Employee was pulling steel out with steel puller. Steel went through top of steel puller into the air, landed on his left big toe, right before the steel tip, fracturing his left big toe.

Employee was trying to thread two pieces of steel together, when they would not go together, he used his percussion to get them to thread and a piece of steel broke off and got wedged under the skin of his chin. Metal was removed from wound and wound sutured closed.

Employee was cutting plastic straps from diamond wire, when knife slipped and cut his left thumb, requiring stitches.

Employee was hammering on a chisel when a metal sliver from the chisel head came off and struck his left arm just below the wrist. Employee was given one stitch and returned to work, no lost time. Employee normally works at the Rockville Shop, was temporarily working at Milbank.

EE put leg up on saw rail, was stripping a wire during a wire change, knife slipped and cut left leg on inside of knee cap. EE received sutures and returned to work, no lost time.

EE was torqueing a side bolt on a doofer hammer, strained right arm. EE didn't see a doctor right away because EE thought it would get better. When EE saw the doctor on 3/30/05, EE received a prescription and also referred for some therapy. EE is still working - no lost time.

Employee was pounding stakes into the ground to hold forms straight. He was setting forms to pour concrete for sidewalk at main office and strained left shoulder. He didn't see a doctor until 9-29-04, then saw a chiropractor a couple of times.

EE WAS MAKING REPAIR TO A DUST COLLECTOR. THE MUFFLER MOVED AND THE HAMMER THE EE WAS USING GLANCED OFF THE MUFFLER AND HIT HIS LEFT LITTLE FINGER, CUTTING IT AND REQUIRING SUTURES.

EMPLOYEE WAS TRYING TO REMOVE A STUCK DRILL ROD WHEN HE HURT HIS FINGER WITH A HAMMER.

EE WAS LIFTING BUSTER TO WEDGE STONE WHEN HE FELT A PAIN IN HIS LOWER BACK. HE SAW A CHIROPRACTOR THAT AFTERNOON AND RETURNED TO WORK ON 5-16-03 WITH RESTRICTIONS UNTIL 5-19-03.

EMPLOYEE WAS TAKING STEEL OUT OF HOLE WITH A STEEL PULLER, THE BIT CAME OFF AND THE STEEL CAME OUT OF THE PULLER AND FELL ON EMPLOYEE'S FOOT, FRACTURING IT.

EMPLOYEE WAS DRILLING AND STEEL GOT STUCK. HE USED A PIPEWRENCH WITH UPWARD PRESSURE TO LOOSEN THE STEEL. WHEN STEEL CAME LOOSE, PIPEWRENCH HIT HIM IN MOUTH, CHIPPING HIS TOOTH.

EE WAS CHANGING BIT ON SLOT DRILL. THE 10' STEEL FELL OUT OF HOLDER, TIPPING OVER AND HITTING EE ON HARD HAT, UPPER BACK AND LEFT ELBOW, CAUSING CONTUSION TO UPPER BACK. Y

EMPLOYEE WAS CUTTING BOARDS ON BOARD SAW. HIS HAND SLIPPED AND HE RAN HIS LEFT THUMB INTO SAW, CUTTING TIP AND REQUIRING SUTURES.

EE WAS CLEANING THE GUIDE ON THE DRILL. HE HIT HIS HAND WITH THE HAND TOOL HE WAS USING TO CLEANCUTTING HIS LEFT MIDDLE FINGER, REQUIRING 4 STITCHES.

EE WAS WORKING ON GROUT TRUCK AND PRYING WITH A PRY BAR. HE STRAINED/SPRAINED HIS RIGHT WRIST. HE DID NOT PAY MUCH ATTENTION TO IT AT FIRST BUT IT CONTINUED TO BOTHER HIM SO HE WENT TO THE DR ON 1-24-01.

EE WAS CUTTING THE END CAP OFF A DRILL STEEL WHEN THE KNIFE SLIPPED AND CUT LEFT HAND BELOW THUMB REQUIRING STITCHES.

EMPLOYEE WAS BREAKING A GRANITE BLOCK WITH WEDGES AND WHEN THE BLOCK BROKE IT TILTED SLIGHTLY, CAUSING EMPLOYEE TO LOSE HIS BALNCE THUS LACERATING HIS RT FOREARM ON THE CORNER OF THE GRANITE BLOCK.

EMPLOYEE WAS WORKING ON A SLOT DRILL HITTING A PUNCH WITH A HAMMER. A PIECE OF THE PUNCH BROKE OFF AND WENT INTO HIS LEFT PALM.

EMPLOYEE WAS WEDGING; WHEN HE GRABBED A WEDGE, A SLIVER WENT INTO HIS LEFT HAND. THIS COMPANY WAS NOT NOTIFIED OF INJURY UNTIL 7/28/00.

EE WAS USING HAMMER TO KN0CK TEFLON OUT OF INSIDE OF DRILL BIT, MISSED THE BIT AND SMASHED RIGHT THUMB, REQUIRING STITCHES.
