Employee was stepping up on grout truck ladder, felt his right knee pop. Went to clinic 7/16/14, was released to work without restrictions. He has again sought treatment for his knee & may now be having surgery, although date isn't set. He continues to work.

Struck against stationary object

Employee was lifting a 12 volt battery out of bracket under the hood of pick-up. When he lifted the batter, his hand hit a sharp piece of metal on the hood and cut his knuckle. Left index finger, second knuckle, sutures.

Fall from ladders

Employee was welding in grout truck box & needed more steel. While going down ladder, ladder slid out from bottom & employee fell to the ground (about 11 feet). Laceration on top of head - two staples, mild concussion; also badly bruised left buttock and left leg.

Employee was lifting 8' long steel I-Beam, putting it in stake pocket hole on saw cart. When pushing end upright, he felt a pop in his lower back.
