Employee was hitting excess granite off saw block using 10 pound sledge hammer. When he hit with hammer, granite bounced off, cutting right side of his face, just above the lip. Sutures placed. No lost time.

Employee was twisting a union onto a pipe, he heard/felt a pop in his left hand/wrist area. Hand began to swell up. He sought medical attention. He again sought medical attention on 4-16-08 as a follow up, was issued a prescription and has restrictions of not using left hand until next week. Is working - no lost time.

Employee was reaching to disconnect the air line supplying air to drill when the end of the hose blew off quick coupler, struck him on right side of face causing bruising and scapes to forehead and side of face. Bruised right eyeball and corneal scratches. Employee received first aid treatment and was issued a prescription - continues to work - no lost time.

EMPLOYEE WAS FIXING GROVE DRILL, WAS HANDING WRENCH TO CO-WORKER AND HIS HAND CAUGHT THE FAN BLADE ON THE CORNER AND HE CUT HIS RIGHT MIDDLE FINGER, REQUIRING STITCHES. THIS COMPANY DID NOT K NOW OF INJURY UNTIL 7/25/00.
