Employee was attempting to close the door on the freight elevator and got EE's hand caught between the inner and outer door, resulting in a fracture to EE's finger
Employee was cleaning the coal elevator and EE dropped EE's hammer into the elevator. EE removed the lower access panel to reach in and retrieve EE's hammer. When EE reached in, the control room operator started the elevator. The employee received abrasions on EE's left forearm.
Hoist tripped power due to incoming power interruption (Cleco) as it was travelling with personnel to 1500 ft level. The brakes engaged, suddenly stopping the skip due to the power trip. Drives were reset and personnel brought to surface. Lost time days started March 31, 2025.
The employee was removing/replacing the guard while the bucket elevator ran. The guard slipped into the opening, getting caught by moving buckets, causing the guard to slam down on employee's lower leg, causing a tibia and fibula fracture.
Employee operating hoist had their right ring and pinky finger smashed after running the hoist block onto their fingers. Injury was not a result of lack of training.
As the guide rope was being installed, the rope slipped and fell to the bottom of the shaft below. When it fell, the tail end of the rope whipped by, hitting an employee on the top of the headframe, causing fractures in the right arm and left ankle.
EE was closing the doors on the passenger elevator and got right hand and middle finger caught between the doors. EE reported the incident on 9/18 as a first aid. On 09/23 EE requested to see a Dr. EE got an exam and X-ray. X-ray was negative. On 09/27 EE went for follow up and was placed on restricted duty because finger looked infected and was given an antibiotic.
Employee placed arm in the path of the closing elevator door.
A Mobile Mechanic boarded the underground mine service cage to leave the mine at the end of the shift. As the man/material hoist started to raise the employee from the mine level, the hoist went into fault and abruptly stopped, throwing the employee to the floor. Once on surface the employee felt pain in lower back and right leg.
Employee removed access cover where sand is lifted using a vertical elevator, attempted to use a piece of rebar to poke at what the employee described as "wet sand" in an attempt to unclog the sand and allow it to pass through to the vertical elevator. While poking at the sand, the rebar contacted a section of the vertical elevator which casued the employee's hand to be pinched.
EE made contact with an elevator bucket
Employee was riding down frontend manlift in GR 3, employee looked up, and got something in their left eye. After an hour, employee started feeling pain in left eye. Went up to guard shack to get left eye washed out. Employee went to clinic, then went to eye doctor. Eye doctor prescribed steroid eye drops due to scratch to eye.
During the descent of the mine cage, ice fell from the shaft wall and struck the protective cover (metal top) of the cage. Abnormally extended cold temperatures resulted in the ice forming in the shaft, in spite of the shaft heaters being used.
We were in man lift boomed out over elevator. A Lhoist employee entered and began operating elevator. The elevator made contact with the man lift boom that I, and my employee, were in. My employee received a broken nose and slight fracture in right knee.
Employee was cleaning a bucket elevator.
Hand in proximity to snatch block while manipulating a belt.
The employee reported feeling something enter EE's left eye while riding down the shaft on the conveyance. The employee reported that EE was wearing safety glasses, but the material entered between the frame of the glasses and EE's face.
A dry plant employee had gone to the head box of bucket elevator 175A to clear an obstruction. When completed the employee had leaned the shovel against the hand rail and started to descend down. Upon starting the bucket elevator the shovel had fallen and struck another dry plant employee on the head. This resulted in a light concussion. Dr. had ordered one day off for recovery.
EE entered a passenger elevator, closed scissors gate, the exterior door did not fully close, EE reached through scissors gate to secure exterior door and smashed right thumb between door and frame.
The designated person called for the cage to pick the people up from the work deck. The designated person failed to notify the hoist person the exact location of the work deck while it was on approach. Therefore causing an employee to be in line of the landing area, the bottom of the cage brushed EE and pushed IE away from landing area.
The elevator an EE was using malfunctioned and stopped approximately 6 ft from the ground level. The EE was stranded in the elevator for approximately 2 hrs. after which, they decided to attempt to exit. As the EE exited the elevator, they fell to the grating on the ground level, an approximate 6 ft fall, striking their side and back. The EE fractured 2 ribs.
A miner was working in the #1 production shaft. While being tied off to a divider, the employee stepped back onto the workdeck and radioed the hoist operator to go up. The employee forgot to transfer the lanyard from the divider to the work deck and was subsequently pulled off the work deck by the lanyard and landed on the divider fracturing a leg.
EE going underground on cage and felt ear pop due to elevation change. EE remained full duty until surgery on 9/22/2020 and was placed off work at that time.
Employee was moving pallets of empty bags up to the bag floor using the 506 freight elevator. EE made three loads with no incident. When EE closed the interior gate, EE felt a pop in the R elbow w/pain. Reported to supv. taken to clinic, diagnosed w/elbow strain. Placed on restricted duty (1 armed) until follow up 3/24.
Employee stated EE was crossing slope cable. EE stated the cable moved and struck EE in the leg. EE finished shift and did not seek medical attention at this time. EE first sought medical attention on 11-25-19. Upon exam, an x-ray was ordered which indicated a fracture. EE has been referred to Orthopedic specialist.
Employee was exiting the preheat tower elevator, after closing inner cage door, the external door started to close as employee was moving out of the door when it closed on the right thumb causing a open fracture laceration.
Employee got right index finger caught between coupler on slope car.
Miner was riding elevator from 3rd floor to 2nd floor. Miner reported that the elevator slipped approximately 4 feet causing miner to fall fracturing left radius.
During first mantrip into the mine, the shaft conveyance came to an abrupt stop.

Ee was riding service hoist when it came to an abrupt stop. Employee flexed knees and ankles to take shock. Resulting in fracture to metatarsal bone in left foot.
Miner was entering an elevator when the door shut unexpectedly hitting their head and causing discomfort on neck.
EE was traveling down the elevator at full speed to the bottom of the shaft. When the elevator reached the bottom it came to a sudden stop resulting in EE's knees to buckle and injure EE's left knee. EE finished shift and went to med express on 4/17/2018 and placed on restricted duty.
The employee was injured while hand loading material onto a freight elevator. The employee fell forward breaking Rt. ulna and lacerating head when the elevator car floor shifted and employee lost balance. Employee was injured on 3/28/18 was referred to Orthopedic Specialist and returned to work with restrictions on 4/2/18.
Employee was clearing bridging wet Aluminum Trihydrate material from bucket elevator inlet chute with a shovel. The area was restricted access with barricade and signage. Shovel got caught to rotating buckets of elevator causing fingers to pinch between the shovel handle and inlet chute. EE received treatment from hospital ER for finger closed fracture and nail avulsion.
Operator was shoveling material into White Rock Elevator in the Dryer Pit The Operator was investigating why the vibrator was not working and grabbed the hose that supplies air to the Vibrator for the Elevator. When the operator grabbed the hose it pulled the lid to the Elevator down onto the operator's wrist. The operator experienced a contusion to the left wrist/arm.
EE and a coworker were on the industrial elevator at Enlow Mine. EE's fingers were pinched between the door of the elevator and the door frame for the elevator door. EE lost the fingernail on and fractured a phalanx in left ring finger.
Passenger elevator was coming down hit boom of manlift and it fell approx 20'
Employee was entering mill elevator and door slammed shut on hand and injured one of the fingers on left hand that resulted in stitches.
Employee was closing the freight elevator door and accidentally got one hand between the doors and closed the door on fingers causing a laceration to right pointer finger.
Employee was closing tower elevator doors not using the grab handle and caught hand between elevator doors when closing, resulting in hand being injured small bone in back of hand fractured.
Elevator 310 locked/tagged out due to blockage, pulled front and back plates to clean blockage. EE was using a bar to remove a rock when the buckets broke loose and pulled the bar and EE's left hand and arm into the machine. EE jerked hand back, realized hand and arm were injured. Went to the office to get help.
Elevator struck employee on left upper thigh causing contusion.
Employee reached under pallet elevator to remove debris and was caught when the lower deck came down. Employee suffered lower face and neck trauma.
The employee was riding in the personnel elevator attached to the 3rd bank silo. The employee complained of lower back pain from the elevator coming to an abrupt stop. The employee did not seek medical attention until 12/14/2015; when he was issued a work restriction (light duty work only).
The injured miner reported pain to his right knee when the Auxiliary cage slowed rapidly.
