We had some containers of sodium sulfide. During a routine inspections it was found that some of the containers had compromised lids. The incident happened when the sulfide was being transferred to new containers. Two of the employees began to throw up and all three were taken to the hospital.

Employee was shoveling under the Z-5 belt, when he collapsed & became unconscious. He was found by his supervisor. The supervisor touched the employee's shoulder & the employee regained consciousness. The employee rested for a while, then requested to be taken home. The employee has a medical condition that makes him weak & tired. This was the cause of his collapse.

Employee was walking around a pallet. The shrink wrap on the pallet had come loose & the end of the wrap was on the floor. The employee did not see it & tripped over it. The employee's knee landed on the forks of the forklift. The employee continued to work with no problems until 12/9/10. He then requested to see a doctor.

Employee was working on an axel off of a backhoe. He had taken the axel off & set it on jacks. The employee checked the axel to make sure it was stable on the jacks. When the employee stood up the axel fell off of the jacks, landing on his hand.

Employee was walking through the shop & tripped over a power cord. When he fell, he turned & twisted & hurt his back. The employee did not seek medical attention, & did not lose any time off of work until 2/7/11.

EE was working on C1 conveyor belt monitoring for debris when a large rock rolled down - he tried to stop it with his hand. EE did not report to anyone that day nor the next. On Monday, he mentioned it to foreman and was immediately sent to make report at office. EE told supervisor that he had played in a basketball tournament over weekend & wrist didn't hurt at that time.

Employee was changing shaker screen. Walking on catwalk he stepped on a spot with a hole, fell through catwalk to mid thigh (left). He knew hole was in catwalk but had not reported to maintenance dept. Tore skin on finger when he grabbed guard rail and scraped left side on guard rail. Tore jeans on catwalk.
