Employee was standing next to wrecker trailer and reached up on the trailer to retrieve EE's earmuffs. When EE stepped back, EE stepped on the welder power plug connection, twisting EE's knee. EE was taken to the ER and given restrictions that cannot be accommodated at this time. Currently ONGOING.
The injured employee stated that while walking behind a scoop EE stepped on a rock concealed by rock dust, causing EE to twist EE's left ankle and sustain a fracture.

On 3/17/26, the employee stepped on a rock and overextended the arch of their right foot. The injury was initially believed to be minor, but the pain persisted and worsened. On 4/13/26, the employee was evaluated at a clinic and prescribed an orthopedic boot. X-rays were negative for fractures; an MRI has been ordered for further evaluation.
Employee stepped down off of motor when EE stepped on a piece of coal and injured EE's ankle. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Operator was assigned snow removal work in the pit. Operator stated after work they started having knee pain. While climbing the ladder they felt pain in their knee. Employee requested to be sent off site and wanted an MRI of their knee. We waited to get the results before filing this report. They got an MRI and reported the results their knee will require surgery.
Mechanic was climbing the stairs on a CAT 777 haul. Three steps up the stairs they felt their right knee pop.

Employee was taking down fire suppression above a scoop charger. When employee stepped off charger EE stepped on rock, rolling EE's ankle. Ankle was found to be sprained and the employee was put on restrictions (mainly sit down work only) and placed in a walking boot.
Employee stepped on a nail. puncturing through EE's left boot into EE's left big toe.
Employee was loading up recovered rail and when EE was getting off car EE stepped down on a piece of rail and rolled EE's right ankle. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Injured employee was dragging a ventilation tube to the right side of the CM for installation. As EE approached the coal pile behind the CM EE set the tube down and stepped across the pile. EE stepped on a loose piece of coal and rolled EE's left ankle. Diagnosed with a ankle sprain.
Walking - stepped on a rock or piece of ice.
The employee was working on hands and knees on and off for several hours and when they stood up they felt discomfort in right knee. Employee was working with other employees changing out plastic coated screens in a shaker deck assembly. No visible bruising, light to moderate swelling with pain realized behind the kneecap. Unable to keep full weight on right leg without discomfort.
Employee was hanging slack for bolter when EE went to walk around machine, EE twisted EE's ankle. PPE was not a factor, mining systems was not a factor, compliance with rule and regulations was not a factor. Body positioning was a factor.
Employee was fueling front end loader and walked away from the equipment and when EE heard the fuel overflowing ran to stop it, stepped on a rock and employee sprained the ankle.
EMPLOYEE WAS STEPPING OFF OF THE FORKTRUCK. THEY STEPPED OF OF THE LAST STEP ONTO THE GRAVEL AND FELT A SHARP PAIN IN THE BOTTOM OF THEIR HEEL. EMPLOYEE WENT TO THE DOCTOR ON 11/06/2025 AND WAS TOLD TO STAY HOME WITH A BOOT ON.
Employee stepped on unknown object at the tail of R4 Ref Belt. The object came up and hit employee in the mouth busting their lip and breaking (3) teeth.
Millwright was working out of a Manlift. They knelt down on left knee and put all of their weight on a bolt that was laying on the floor of the basket. This caused pain in their knee. This happened on 10/14/25, they did not report it to employer until 11/5/25. Once they sought medical they were sent to Physical Therapy on 12/17/25 which at that time made it a reportable incident.
on morning of 10/12/2025 at around 10:00am employee stepped out of truck, stepped on rock and said EE's knee popped. EE finished shift and went to the doctor the next day
While exiting mill building IE stepped on a grinding media ball and sprained EE's ankle
torn meniscus in the right knee from crawling. EE Never missed any work until today 12/4/25 because EE's surgery is today. EE worked on light duty until today. Again NO work missed until now.
Employee was walking along belt drive when EE stepped on a nail. PPE was not an issue, mining systems was not an issue, compliance with rules and regulations was not an issue.
Employee was working in the milling room while concrete wall was being removed. Employee stepped on a wedge and rolled ankle. Employee informed company on 10/1/2025
Employee stepped onto driver's side tire to grab the vehicle chocks from the bed of the truck. While stepping off of the tire, the employee placed EE's foot onto a loose rock. When the rock spun out beneath the employee, the employee felt their right knee "pop".
Employee rolled EE's ankle after EE stepped on a 2" hose that was laying on the concrete floor after stepping down from the crossover platform steps. Medical professionals were notified and employee released to full duty. On 9/27/25, employee felt discomfort return and advised EE to get an evaluation at a medical facility where EE was diagnosed with "other fracture of the fibula".
Employee was carrying a stick of water line with another miner when EE stepped on a piece of slick line and injured EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee rolled ankle at drill pattern
Injured employee was walking along the shuttle car haul road on the 8W CM Section. EE heard a shuttle car approaching and hurried to get in a cross-cut. Stepped on a lump of coal and rolled EE's left ankle. Diagnosed with an ankle sprain. Did not violate any rules or regulations.
While descending the screener stairs, the operator was carrying a chain hoist in one hand and using the other to hold the handrail. As they reached the bottom of the stairs, the operator stepped onto a hose that was lying across the walkway, causing them to twist their right ankle.
EE stepped out of a man basket and rolled EE's ankle after stepping on a rock. Went to Concentra, no lost time or restrictions, but was given crutches and a brace. Returned to work the following day.
IE was walking in the belt entry and stepped awkwardly on a small rock. IE suffered a fracture on the outside of the right foot.

On 08/23 IP reports climbing into engine of truck and stepped on a hose. Foot slipped and left ankle rolled laterally. IP completed shift but noticed left foot swelling and pain. Reports going to Riverton ED, had x-rays done which were reportedly negative and was given crutches and rocket sock. Was sent in by employer to RMMC on 08/27.
Miner was kneeling down on concrete floor performing clean-up activities and injured knee.
