Accident type, without injuries

Haul truck 3175 dumped a load at the leach pad. After dumping, an employee noticed that 3175 had started leaking fluid. While descending the leach pad ramp, the driver of 3175 observed flames coming out from under the deck over the engine. The driver stopped the truck and safely exited the haul truck.

Employee was surveying blast hole collar when survey rod hit something causing it to twist and tip back against thumb.

Employee states EE was exiting the shift change van at the ready line when EE stepped out onto a rock and rolled EE's foot, falling and heard a pop in EE's left foot.

Loader 3222 was loading haul truck 3179 in the J2 Pit. As the loader approached the truck to dump a bucket, a rock rolled off the right side of the bucket and contacted the left bedrail, causing the truck move. The haul truck driver reported back pain and requested to be evaluated in the emergency room. Worker was placed on restricted duty for the rest of the week.

On 4/7 while tightening a lug nut the impact wrench twisted workers right arm and slipped out of their left hand. Worker didn't report injury until 4/8. Declined medical evaluation at that time. Worker continued full duty. 4/18 the worker took themselves to a specialist without company knowledge. The injury became reportable on 4/21 when they reported to work with restrictions.

Survey supervisor was opening the door to pickup when their pinky locked up and they jammed it on the door when grabbing the handle.

Temporary employee was accessing pickup after lineout meeting. As the worker approached the vehicle, they misplaced a foot in a parking ditch, resulting in an ankle sprain. Worker was provided with a splint and placed on modified duty.

On November 14th, a mechanic bumped their knee while descending the back of a maintenance truck. Knee became swollen and painful. The employee continued working full duty. On December 3rd the injury became reportable when the employee requested medical evaluation at local hospital and an immobilization brace was applied, and potential fracture was diagnosed.

Mechanic was tightening the fitting on a steering cylinder when the wrench slipped hitting mechanic in left front tooth and causing a chip to the tooth.

While walking to a restroom at the fuel skid, a worker caught the tip of their toe on a clump of dry mud and fell forward. The worker extended their arms to brace as they fell. When they landed, they injured their left wrist. X-rays confirmed left radius distal fracture.

The injured Driller Assistant was guiding a piece of 5-7/8"" casing being lifted by the wireline to install when EE's left hand was caught between the casing EE was handling and the drill table. The middle left finger received smashing laceration and broken finger tip.

7/2/2023 While working in the pulverizing room employee was splitting samples and felt a pop in EE's left wrist. The employee did not wish to seek medical attention at the time of injury. On 7/5/2023 employee was seen by medical professionals for continued pain and was placed on restricted work assignment.

On 10/19/21, employee hammered a loader tooth pin. Metal came off, struck the employee's cheek, causing a cut. First aid was provided. Injury became reportable 11/11/21 when employee requested medical attention for prolonged irritation in the wound. Foreign metal body was removed and sutures were applied.

When a site maintenance technician lit a torch, they heard a pop under their arm. The oxygen hose had burst, resulting in fire, causing first and second degree burns to the worker's upper arm and back.

Employee was connecting a water hose onto a fitting. Employee felt a pop in right elbow when connecting the hose. Employee was placed on modified work assignment due to elbow strain.

Employee tripped on a hose or rock while refueling EE's haul truck at the waste dump. Employee fell against the containment and struck EE's head, resulting in contusion.

Employee jarred back when loading a dozer onto an over the road transport trailer. Employee did not wish to seek medical attention at that time. On September 1, 2020 the employee asked to be medically evaluated because back still hurt. Employee was placed on restricted work.

Employee was training in the water truck, riding in the passenger seat. The trainee placed a hand on the door post to adjust position in the passenger seat. At that time, the trainer closed the door, catching the tip of the trainee's thumb in the hinged door jam, causing crush injury that included fracture and cut.

Employee forgot to step down when exiting a portable rest room, resulting in a fall. Employee strained right shoulder and initially returned to work without seeking medical evaluation. The next day, July 8th, the employee requested to be evaluated. Restricted duty was assigned.

Operator was checking oil when a loose strand of wire on the dipstick penetrated employee's finger. Employee was placed on modified duty with a splint.

Shift began at 6:00 AM, employee was staking the top of shot muck. Employee tripped over loose rock, twisted right knee, resulting in burning sensation in the knee. Remained first aid injury (non-reportable) with worker working full duty from date of occurrence (1/22/2020), until worker began receiving physical therapy on 5/18/2020. Worker continues to work at full duty capacity.

Employee stepped on rocky area and rolled left ankle. Treatment was not sought until October 5, 2019 when diagnosed with a strained Achilles tendon. This became reportable on October 6, when employee began restricted duty.

When stepping out of pickup truck, employee twisted EE's left ankle as EE placed EE's foot on the ground.

Over-exertion in pulling or pushing objects

Employee was manually sliding a vehicle gate shut. While pulling the gate, EE felt a pop in the back side of EE's right knee, followed by pain. Soft tissue injury (strain resulted). Specific diagnosis is pending results of an MRI.

Fall down stairs

Employee was descending stairs at carbon columns and a heel caught on the second step from the bottom of the staircase, causing the right ankle to roll. Employee attempted to catch themselves with the left foot and rolled the left ankle in the process. Both ankles were sprained and x-rays were positive for broken bones.

On 10-11-2018 an individual was found unresponsive due to what appears to have been a fatal personal medical health condition.

On 10-14-17 employee began feeling pain in right knee and limping. No specific event contributed to the pain. Employee did not seek medical attention until 11-1-17. Event became reportable on 11-2-17 when employee began modified duty.

Fire started in the median on Interstate 80, presumably from a cigarette thrown out of the window from a commuter. The fire crossed eastbound lanes and spread onto the mine site where it burned for approximately one hour before crews extinguished the flames. Employees were evacuated as a precaution. No injuries occurred.

While unloading sandbags from the back of a pickup, a laborer was injured when the driver of the pickup backed up without warning, striking the victim's abdominal area with the tailgate. This resulted in abdominal contusion, causing lost work days.

A mechanic rolled a lube truck onto its side when ee lost control due to traveling too fast for conditions. The mechanic sustained cervical strain, lumbar strain, and head contusions, resulting in restricted work activity.

Portable diesel heaters were been brought into the shop to provide supplemental heat. A lube truck was also running inside of the shop. A mechanic became ill due to carbon monoxide poisoning, which resulted in medical treatment.

Employee had to climb a fixed ladder on tank to obtain tank product level information. As employee descended the ladder, foot slipped off step and employee fell 3.5 feet to the ground. Fall resulted in a right calf muscle tear and right shoulder strain.

Employee was in the process of servicing a 785 Cat Haul Truck and as he stepped onto the deck and did not have three points of contact, which was available, fell approximately 11 feet to the ground sustaining muscle bruising.

Preparing to unload, truck extending hose out, when pulling on the hose my foot hit an unsafe surface (a pile of dirt) tripped backwards extended my arm to catch my fall. I fell hard. That is when I broke the bone in my wrist. I informed shift manager at Florida Canyon mine and the safety guy. Did all incident reports with help from a girl.

The object rotated slightly when placed in the pickup bed and a metal splinter from the threads punctured his right ring finger. A small piece was lodged in his finger. Employee was in the process of removing the fuel wiggens fitting from the loader e-z valve. The e-z valve stuck open causing diesel fuel to spray in employee's right eye. This is a reportable due to prescribed medication.

Employee reported he was standing at the fuel island waiting for the truck tank to fill. He stated he shifted his body weight from his right leg to his left leg and at that time his left knee give out.

Employee lost footing and fell while exiting after checking motor grader engine motor oil level. Employee sustained a broken nose and injured knee due to the fall. Dr. prescribed light duty only.

Employee was assigned the task of numbering and collecting approximately 200 blast hole drill samples each weighing between 1 and 5 pounds. As he was traveling back to the sample lab he noted his back began to hurt and get stiff. By the end of the shift the pain and discomfort remained and he reported his condition to his supervisor.

Employee was squatted down for about 30 minutes installing a hydraulic hose on a loader. When he attempted to stand up he was unable to straighten his left knee or put any weight on his leg.

EE was asked to inspect a haul truck tire (3300X51). EE noticed the tread leaving casing. EE installed a spare, the scrap tire was laying on the ground. EE released the tire hand from the tire, a flap of rubber then hit EE on the left knee.

Employee was descending the driver's side ladder of A Cat 785 B haul truck when he lost his footing on a ladder rung and fell backwards twisting his right ankle.

Employee was removing ball joint from steering assembly, heat was applied to allow removal of ball joint. After removal ball joint fell to concrete floor, after approx. 30 sec. joint housing failed causing metal cap to fly approx. 10 ft. & strike employee on right shin causing laceration that required 3 suture to close.

Employee was walking past the anti-scalant tank at the Processing Plant. Employee slipped on the ice, fell onto his back, causing a strain to the upper back.

A haul truck contacted the rear tire of the motor grader shoving it to the left. The event caused the motor grader operator to be jolted in his seat straining his right shoulder and left arm. Employee refused medical attention until five days after the event.

Pouring concrete with a hose, the form filled up to knee level. Washed clothes off, water went down into work boots burning legs & feet.

Serrating concrete as it was being pumped from the truck into the forms. Was standing in the form, concrete splattered on legs. Washed off clothes, but didn't wash skin.

Employee was attempting to jump across an approximate three foot wide water puddle when he slipped and fell. The fall caused a contusion to his right cheek and right wrist.

Welding rebar for wash bay, became overheated. Due to diabetes blood sugar went high, was not able to bring it down.

While the ATV was in motion an unsecured rope fell of off the equipment rack and became bound around the rear tire causing the ATV to skid. The rider was thrown off the machine fracturing his left knee.

The employee was pulling a steam hose off of a steam truck and hyperextended his left elbow. The elbow is currently strained and the impairment is unknown at this time.

At approximately 3:20 AM, 1/17/07 the propane vaporizer building was observed to be on fire. The vaporizer facilitates a 20,000 gallon propane tank which services the Process Plant. There were no injuries and the fire was extinguished with only paint damage to the small metal building. Damage to the vaporizer has not been determined as yet

Employee was working the south ditch project, cleaning the ditch. He bent down to lift a load of mud and vegetation and popped his upper back out between his shoulder blades. He lifted too much weight with his shovel.

Employee was in the process of installing an arm rest cover on a loader seat. While pushing and pulling he strained his stomach muscle.

Employee was ascending the left ladder of a 785B Cat Haul Truck and lost his grip falling backwards to the ground. He sustained a fractured right wrist and two compression fractures to l1 and T 12.

Employee fell stepping off the running board (steps) of a lub/oil truck. As he fell he struck the back of his head on an adjacent parked lub/oil truck. He stained an approximate one-inch laceration to the back of his head.

Employee slipped on a tooling wrench which was lying in the walkway. He stopped his fall by grabbing the hand rail. The fall caused him to ""pull a muscle"" in his lower back.

Employee parked fuel/oil truck to adjust the rear brakes. After completing the adjustment the truck rolled over the wheel chock. The employee ran behind the moving vehicle slipped falling to the ground on the left side of the vehicle. The left front tire rolled over his left ankle causing a fracture to his left ankle and left lower leg.

Employee was removing a nut from a 992 loader bucket wear plate using an air impact wrench and socket. The socket came off of the wrench striking the employee's left index finger tip causing approximately a quarter inch laceration.

Haul Truck Driver recieved back injury while being loaded by a 992C loader. Loader bucket hit the side of the truck and jolted the employees back.

EE performing nonstrenuous labor collasped. Immediate first aid given. EE was transported to Pershing General Hospital where he was evaluated to have heart related complication not attributed to work. Not reported because incident was not a work related injury.

WATER TRUCK OPERATOR ATTEMPTED TO CLIMB A STEEP RAMP IN THE RADIO TOWERS PIT. THE TRUCK FLIPPED OVER BACKWARD AND CAME TO REST ON ITS TOP. THE OPERATOR RECEIVED A LACERATION TO THE TOP OF HIS HEAD AND CONTUSIONS TO HIS RIGHT SHOULDER.

EE WAS REPAIRING THE BIRD NETTING ON THE S-1 SOLUTION POND WHEN HIS FOOT BECAME ENTANGLED IN THENETTING AND HE FELL TWISTING HIS RIGHT KNEE. INITIALLY HE DID NOT SEEK MEDICAL ATTENTION BUT WHE N PAIN IN KNEE DID NOT IMPROVE HE WAS SEEN BY A DOCTOR IN AUGUST AND ORTHOSCOPIC SURGERY WAS PERFORMED ON 9/20. HE RETURNED NEXT SCHEDULED SHIFTWITH NO DAYS LOST, LIGHT DUTY ONLY.

EE WAS CLIMBING LADDER OF 777 WATER TRUCK AND STRUCK HIS KNEE ON THE BUMPER. THE FOLLOWING DAY HE WAS SEEN BY A DOCTOR AND DUE TO EXTREME SWELLING IT WAS NECESSARY TO DRAIN THE FLUID FROM HIS KNEE.

EMPLOYEE WAS REMOVING A CHOKER FROM THE MAIN HOIST ON 90 TON CRANE. ANOTHER EMPLOYEE WAS HOLDING THE SAFETY LATCH OPEN. HE RELEASED THE SAFETY LATCH BEFORE INJURED EMPLOYEE HAD HIS HANDS CLEAR AND LATCH SMASHED TIP OF RIGHT RING FINGER. INJURY REQUIRED 6 STITCHES TO CLOSE.

EMPLOYEE WAS LOWERING A HEAVY POWER CABLE FROM ACONVEYER FRAME USING A CHAIN WHICH SLIPPED. THE CABLE FELL AND THE EMPLOYEE'S FINGER WAS PINCHEDBETWEEN CHAIN AND CONVEYER FRAME. WHEN HE PULLED HIS FINGER OUT OF THE PINCH POINT HE RECEIVED A LACERATION WHICH REQUIRED MEDICAL TREATMENT (SUTURES).
