Around 8AM EE was working at the crusher. EE slipped on ice. Injured EE's back.

Employee walked to the shop to load parts into the company's truck. They said they started to feel funny. They passed out and fell on their back and hit their head on the ground. Ambulance was called.

Walking - stepped on a rock or piece of ice.

Injured left thumb. Was hooking a slab.

EE was hooking a wire cable around a block of stone at the mill. The wire cable was hooked to an overhead crane. The block of stone was sitting on a 2"" rock skid. EE was using right foot to push the wire cable under the rock. The crane operator thought heard EE was done and started up with the wire cable to raise the rock. EE's right foot was caught between the wire cable & rock.

Climbed off loader stepped on rock and fell on left shoulder.

Got right hand caught in the belt of the pitching machine. They had to cut the belt to get hand out.

Swinging hammer. Thinks pulled right shoulder.

As A bundle of scrap stone was being set down to be dumped, employee had gotten their right thumb between two pieces of stone.

Fell off a ladder.

The employees were getting ready to wash the ledge with a water hose when they turned it on they didn't know it had some ice in it. The hose stopped up and caused pressure to build up in the hose. The employee that got hurt couldn't control the hose & got struck in the right eye. Happened fast. Nobody knows if the hose hit them or if they hit their eye when they fell.

EE fell off side of loader. Height from catwalk to ground is 7'. It happened on top of S-Ledge. EE was running B-Model loader moving block of stone. At some point the ladder on left side of operator has been hit against something. EE went to exit loader down this ladder & it broke off the side of loader & EE fell to ground onto rt. side. EE was on top step about 6' from ground.

Sliding limestone treads to bundle.

Fell backwards while trying to move an attachment for the skid steer by self.

Employee picked up a rock and injured his back. Went to IU Health Occupational Services for medical review. IU Health gave him medication and requested to return in 7 days or sooner if problems worsen. Employee will be off work for 7 days. The date of the injury per employee was 9/22/15. Reported to management on 10/1/15.

Pulling out splitters. Left side of stomach.

Lifting guard on saw.

Rolling a discharge hose.

Pulling saw pump from quarry hole.

Picked up stripping hose and injured lower back.

Block fell back. EE landed on knees.

Piece of wedge came off and hit him in right forearm and cut a small chunk of skin.

Working on forklift, laying on side. Strained neck while holding head up.

Hit right wrist with hammer.

Swinging hammer injured right shoulder.

Twisted left thumb in belt when moving rock.

Stepping off pitching machine twisted left knee.

Something pulled in groin. At the time of injury he was scraping chutes on crusher.

Employee was dragging a 10"" pump hose. Hurt lower back. Restricted work duties as instructed by the doctor.

Stepped off saw track. Fell on ledge.

Lifting cross tie. Hurt lower back.

Hit head on track. Cut above right eye. Fractured right arm.

Sprained ankle.

Sprain right ankle. Right ankle started to hurt while bedding cut.

Smashed left middle finger between 2 hammers.

Taking key out of Bobcat. Got foot caught in safety chain and fell backwards and layed on tongue of generator.

Pulled muscle in left shoulder area. Pulling a water hose.

BEE STING TO UPPER LIP.

USING A BREAKING HAMMER JONATHAN GOT PIECES OF STEEL IN UPPER RIGHT ARM. HE WAS HITTING A WEDGE WHEN TOP BROKE OFF STRIKING IN RIGHT UPPER ARM.

MOVED SAW CABLE. INJURED RIGHT SIDE. HERNIA. PRIOR CONDITION.

ROCK FLIPPED OUT OF LOADER TIRE AND HIT EE IN THE LEFT SIDE OF HEAD AND INDEX AND MIDDLE FINGERS ON LEFT HAND.

EE WAS LIFTING AN AIRBAG. HE FELT A PAIN IN HIS RIGHT TESTICAL. HE SAID THAT HE WAS OK. HE DIDN'T NEED TO GO TO PROMPTCARE. ON NOV 2, 2000 HE SAID HE WANTED TO TO TO PROMPTCARE TO BE CHECKED B ECAUSE HE GOT UP FROM BED AND WAS IN A LOT OF PAIN.
