Around 8AM EE was working at the crusher. EE slipped on ice. Injured EE's back.

Employee walked to the shop to load parts into the company's truck. They said they started to feel funny. They passed out and fell on their back and hit their head on the ground. Ambulance was called.

Caught in, under or between a moving and a stationary object

EE was hooking a wire cable around a block of stone at the mill. The wire cable was hooked to an overhead crane. The block of stone was sitting on a 2"" rock skid. EE was using right foot to push the wire cable under the rock. The crane operator thought heard EE was done and started up with the wire cable to raise the rock. EE's right foot was caught between the wire cable & rock.

As A bundle of scrap stone was being set down to be dumped, employee had gotten their right thumb between two pieces of stone.

The employees were getting ready to wash the ledge with a water hose when they turned it on they didn't know it had some ice in it. The hose stopped up and caused pressure to build up in the hose. The employee that got hurt couldn't control the hose & got struck in the right eye. Happened fast. Nobody knows if the hose hit them or if they hit their eye when they fell.

Fall from machine

EE fell off side of loader. Height from catwalk to ground is 7'. It happened on top of S-Ledge. EE was running B-Model loader moving block of stone. At some point the ladder on left side of operator has been hit against something. EE went to exit loader down this ladder & it broke off the side of loader & EE fell to ground onto rt. side. EE was on top step about 6' from ground.

Employee picked up a rock and injured his back. Went to IU Health Occupational Services for medical review. IU Health gave him medication and requested to return in 7 days or sooner if problems worsen. Employee will be off work for 7 days. The date of the injury per employee was 9/22/15. Reported to management on 10/1/15.
