AT APPROX. 10:45 AM THE LOADOUT "TRAIN" WAS STARTED. EMPLOYEES WERE EACH TO PUSH COAL TO THE LOADOUT CHUTES. AT APPROX. 10:50 AM AN EMPLOYEE NOTICED SMOKE COMING FROM THE OPPOSITE SIDE OF THE TUBE. WHEN HE INVESTIGATED HE FOUND DOZER IN THEHOLE. HE REQUESTED LOADOUT TO STOP. COAL COVERED DOZER. AT APPROX. 12:30PM DOZER PULLED OUT. VICTIM IN SEAT W/ BELT ON. RESUSCITATION FAILED.

AS THE EMPLOYEE TRAVELED THROUGH THE SECO ND DOOR IT WAS ACTIVATED BY ANOTHER PASSENGER ON ANOTHER PERSONNEL CARRIER ABOUT 140 FEET INBY THE DOOR. AS THE DOOR CLOSES, THE BOTTOM EDGE STRUCK HIM, PINNING HIM AGAINST THE FRAME OF THE P
INSTALLING ROOF BOLTS ON SECTION.INSERTING RESIN AND BOLT INTO HOLE WHEN ROOF ROCK FELL STRIKING HIM ON TOP AND BACK PUSHING HIM INTO BOLTING MACHINE.THIS CAUSED INTERNAL CRUSHING INJURIES.

THE EMPLOYEE WAS CAUGHT BETWEEN THE CABLE ARM AND THE RIB CAUSING HIS CHEST TO BE CRUSHED WHICH CAUSED FATAL INJURIES 001 SECTION NO. 7 ENTRY.
VICTIM WAS RESTING AGAINST BOOM OF HIS PINNER WHEN A LOADED SHUTTLE CAR HUNG INTO PINNER CABLE (HUNG ON CROSS OVERS) DAMAGING CABLE TO EXTENT THAT ALL THREE CONDUCTORS & C ROUNDS WERE SEVERED A PHASE CONTACTED A GROUND TO FRAME OF PINNER ENERGISEING
FATAL VICTIM AND TWO OTHER MEN REACHED DOWN TO PICK UP MINER CABLE WHEN A PORTION OF COAL RIB AND CAP ROCK FELL SUDDENLY CRUSHING AND PINNING THE VICTIM.
