INSTALLING ROOF BOLTS ON SECTION.INSERTING RESIN AND BOLT INTO HOLE WHEN ROOF ROCK FELL STRIKING HIM ON TOP AND BACK PUSHING HIM INTO BOLTING MACHINE.THIS CAUSED INTERNAL CRUSHING INJURIES.
