While replacing the bit bearing on DTH the piston snap ring broke allowing the piston fall causing a crushing injury to right small finger

Employee believes they were grinding clamps on a belt when something went behind their PPE and got in their eye.

Employee turned off the agricultural lime plant to dislodge a rock in the tail pulley. After locking of the conveyor he dislodged the rock. When he did that, gravity moved the conveyor belt entangling employee's arm in the tail pulley.

EE was at the top plant of the rock quarry changing screens. As he was changing a screen, his finger was smashed between the screen & vibrator.

Miami County Highway truck pulled up to stone pile behind Rock Industries Hauler truck. Rock Industries EE was unaware that the County truck pulled in behind him and backed into the passenger side of the County Truck. No injuries occurred. The County truck had cab damage to the passenger side.
